Job summary

Rennie Grove Peace Hospice Care services are provided 24 hours a day, 7 days a week. The post holder will be required to work across shifts covering 9:15pm - 7:15am as part of an extended community team working in the patients' home environments including, as required, in care homes. Although based in the community, post holders may occasionally be required to work in other Rennie Grove Peace Hospice Care settings in order to ensure adequate staffing levels and to gain experience of the wider hospice delivery of care.

Main duties of the job

Proactively contribute to the delivery of high-quality safe evidence-based care. Responding to SOS calls visiting patients in their own home to provide palliative and end of life care. To lead on and co-ordinate assessment, planning and evaluation of individualised patient centred care. Ensuring a holistic approach that is sensitive and responsive to the patients' ever-changing needs. To be responsible for the co-ordination and smooth running of a shift and visits ensuring it is well organised. As a member of the wider Rennie Grove Peace Hospice Care staff sign up and demonstrate delivery of the organisational agreed mission, vision and values. Acting as a Rennie Grove Peace ambassador within the community. As a lone worker be responsible for your own and others safety by adhering to the Lone Worker policy and procedure requirements.

We provide care and support for adults and children diagnosed with cancer and other life-limiting illness, and their families, visiting day and night. Job responsibilities

To undertake holistic needs assessment that explores individual patients and their carers/families physical, emotional, spiritual, practical, welfare right and if appropriate the families' pre and post bereavement need, using a variety of tools to support the process. To be able to develop, update and monitor the individualised care plans ensuring they are patient centred and support the identified patient needs, including ongoing referrals to other services as required. To have the highest regards for the principles of confidentiality and the importance of patient consent, ensuring adherence to the hospice data collection and patient consent policy. In accordance with all hospice policies, best practice guidelines and clinical protocols, ensure own and others' practice is safe and effective. To demonstrate ongoing personal and professional development in accordance with the agreed wider Patient Services goals and agreed individual appraisal objectives. To be an exemplary role model to all peer colleagues, new staff and visitors to the service. Person Specification

Qualifications

Essential

Registered general Nurse/Paramedic. Relevant postgraduate qualification relating to speciality or evidence of transferable experience.

Desirable

Mentorship course or willingness to undertake mentorship responsibility.

Experience

Essential

Experience of shift/caseload management.

Desirable

Experience of working within palliative care/long term conditions/in an oncology setting. Experience of participating in clinical audit/quality management. Experience of leading and motivating teams of volunteers.

Additional Criteria

Essential

Able to build good working relationships. Able to communicate effectively both verbally and in writing. Good administrative and record keeping skills and able to work with good attention to detail. Strong organisational skills. Sensitivity and understanding to the needs of the volunteers and customers, many of whom may have experienced Rennie Grove Peace services or be bereaved themselves. Be a perfect match with our values. Understanding of current palliative care matters. IT literate and experience of using an electronic patient record system.

Strong problem-solving skills and able to implement new initiatives.
