Are you an experienced AHP, RGN or HCP ready for an opportunity for development into advanced clinical practice?


The Directorate is pleased to offer an exciting opportunity for an enthusiastic Clinical Practitioner to work as a member of the Urgent Community Response team to support the Coventry Community Out of Hospital offering and wider admission avoidance strategies.


The successful applicant will be working as a Clinical Practitioner supporting the Urgent Community Response pathways to reduce unnecessary hospital admissions and contribute to effective admission avoidance.

The role incorporates the use of high-level health assessment skills and use/willingness to work towards the non-medical prescribing qualification to support patients from a medical perspective presenting with a wide variety of acute medical conditions within the community setting.


You will be supported to develop advanced clinical skills and reasoning, gaining experience of working alongside highly specialist Advanced Clinical Practitioner staff within a robust MDT.

Access to HEE funding will support investment into your development and access to Advanced Health Practice learning.

You will be required to demonstrate critical thinking in the clinical decision-making process; identify gaps within the service and assist with service improvement and development in line with national and local strategies.

You will be responsible for facilitating effective teamwork and provide mentorship; teaching and support across the multidisciplinary team.


The post holder will be required to initiate effective history taking and high-level clinical assessment to form a clinical diagnosis to support the implementation of a range of medical and care interventions for patients presenting with acute conditions.


The post holder will also be required to support clinical telephone triage on a rotational basis for all referrals received into the service to ensure safe and timely signposting to the most appropriate clinician or service to best meet patient's needs.

To participate in 7 day working rotas to ensure delivery of key services.
