The post holder will demonstrate critical thinking in the clinical decision-making process.


They will work collaboratively with the Emergency Department team, Acute medicine teams and in-reach specialities to meet the needs of patients and will play a key role in service delivery and adherence to policy and procedures.

They will also be instrumental in guiding junior medical and nursing staff in relation to relevant protocols and procedures.


They will be instrumental in providing relevant education and training for nurses, junior doctors, other members of the multidisciplinary teams and analysing ongoing training needs.

The post holder will be required to ensure evidence-based care is delivered to patients and will be involved in audit of practice, research, and service improvement.

Clinical

  • Coordinate the care of the Hospital at Home patients throughout the care pathway with the multidisciplinary team and clinical services to ensure the effective management of this group of patients.
  • Accountable for own actions in accordance with the Code of Professional Conduct. Will contribute to corporate objectives, acting within local, Trust and statutory guidelines and policies always.
  • Highly developed communication skills enabling maintenance of highquality service within a complex and challenging environment with often, conflicting priorities.
  • Use advanced clinical assessment skills to autonomously manage patients with complex needs presenting to the hospital with an undifferentiated diagnosis to ensure that optimum physical and psychological care needs are met.
  • Proactively develop treatment plans for patients requiring urgent intervention including fluid resuscitation, oxygen therapy, antibiotics or other urgent treatment
  • Undertake procedures such as cannulation, venepuncture, arterial blood gases to facilitate efficient urgent care in the context of acute illness or sudden deterioration.
  • Initiate relevant investigations such as Xray, Ultrasound, ECG and blood tests to enable timely diagnosis and facilitate early decision making.
  • Prescribe and review medication when approved by the NMC which are appropriate to patient needs in the context of acute/emergency illness, taking into account any treatment given previously for preexisting medical conditions. Providing information and advice to patients on prescribed medication regimens, sideeffects and interactions supported by the Advanced Nurse Practitioner and Clinical Teams as knowledge and skill develop.
Communication


Document clearly all aspects of assessment, diagnosis and management plan clearly and present the findings to the Hospital at Home Consultant or Consultant nurse when required.


  • Communicate complex information effectively to patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating.
  • Maintain effective communication with the multidisciplinary team within Hospital at Home and with other departments Use the SBAR approach to communicate clinical concerns to the senior medical staff in the event that a patient is found to be critically unwell or deteriorating clinically
  • Act as an advocate for patients and colleagues
  • Ensure awareness of sources of support and guidance (e.g., PALS) and provide information in an acceptable format to all patients, recognising any difficulties and referring where appropriate.
