We have an exciting opportunity at the Royal Orthopaedic Hospital for a Band 6 Critical Care Outreach Sister/Charge Nurse

Main duties of the job The post holder will be expected to:
  • As a skilled member of the Critical Care Outreach Team, provide early interventions with appropriate assessment, observation, and treatment for acutely deteriorating patients in the trust environment.
  • A credible resource in the field of Critical Care/Acute Care who can provide extensive clinical advice and support to ward based staff, to enhance their knowledge and skills in caring for the acutely/critically ill patient in the ward environment.
  • Support the rapid identification and treatment of patients with suspected and/or confirmed sepsis.
  • Provide a key role in assessing and assisting in acute pain management plans ensuring all trust standards are maintained and monitored.
  • Follow up and assess patients being discharged from a level 2 environment to level 1 environment.
  • Assisting with intra and inter hospital transfers of level 2 and level 3 within operational hours
  • Form part of the resuscitation team as an advanced life support provider.
  • To develop the service through clinical expertise, leadership, education, audit and quality improvement.
  • Establish and maintain professional networking relationships with the multidisciplinary healthcare team across the Trust.

Clinical Practice
  • Be able to participate fully in referral systems i.e., NEWS2, allowing healthcare staff to seek advice on the case of the acutely ill, deteriorating patient.
  • Assess, plan and implement and evaluate individualised patient care, providing support and advice to medical, nursing staff.
  • To assist in the discharge of patients back to general ward environments, ensuring a seamless transition of care.
  • Undertake practical skills as necessary to ensure optimum patient management including advanced patient monitoring and equipment management.
  • Undertake a wide range of advanced clinical skills where appropriate training has been completed and competency achieved.
  • Manage expected and unexpected clinical events requiring unpredictable levels of physical effort according to patient dependency/clinical need.
  • Participate in the management of haemodynamic instability and provide appropriate respiratory and vital organ support.
  • Supporting patients, carers and staff during difficult situations arising in the clinical area i.e., delivering bad news.
  • Undertake daily ward rounds providing advice to medical, nursing staff and initiate treatment changes as necessary.
  • Form part of the acute pain service, reviewing patients with epidurals and supporting the wards with the use of PCAs.
  • Act as an expert resource for cardiac arrest prevention and resuscitation related issues and actively participate in the management of cardiac arrest and medical emergencies.
Management and Personal
  • Act as an autonomous practitioner, who directs, provides and evidence-based patient care founded on nursing and medical models.
  • Act as a professional role model that leads by example in relation to attitudes, conduct and appearance.
  • Develop inter-professional collaboration and work in partnership with ward-based staff across Trust.
  • Participate in the clinical/nursing follow up of patients discharged from the High Dependency Unit.
  • Inform practice development activities by using clinical risk evaluation, complaints, and indent reporting.
  • Provide clinical and professional advice and support to ward-based staff as required.
  • Manage available resources within financial constraints to achieve optimum patient care.
  • Maintain appropriate documentation in accordance with agreed procedures and national standards.
  • Participate in the multi-disciplinary review of patients on general wards, including those patients who have been referred to the rapid response team and have remained on the ward and those who have recently been discharged from HDU.
  • Liaise with the appropriate clinicians and organise the admission of patients to a critical care bed where appropriate including the Adult Critical Care Co- ordination Transfer Service (ACCOTS)
  • Participate in the resuscitation of patients as a cardiac arrest bleep holder.
  • Comply with the requirements of the Health and Safety regulations to provide a safe environment for the patients, their families, and members of staff.
  • Maintain data collection systems in order to provide information to enable identification of trends to aid decision making processes and to facilitate audit.
  • Develop working partnerships with outreach nurses on other sites within the Trust and within the Network.
  • Be able to deliver high standards of patient care in order to comply with clinical governance.
  • Participate in the development and implementation of guidelines including sepsis and acute pain for staff to assist in the identification and care of patients who may be at risk of deterioration during their stay at ROH.
Education and Training
  • Inform and educate Trust personnel about the existence and purpose of the Critical Care Outreach Team
  • Contribute to the Critical Care training day for clinical staff equipping them with the skills and knowledge to identify and manage patients who are at risk of deterioration.
  • Assist in the formal and informal education and training of members of staff on general wards who are caring for acutely ill patients out of the critical care environment.
  • Actively participate in and develop seminars and teaching, both formally and informally within the clinical environment.
  • Deliver resuscitation training in line with Resuscitation Council guidelines.
  • Education and training of acute pain management.
Research and Development
  • Act as a change agent and support other staff in the implementation of new practices.
  • Work in collaboration with the multi-disciplinary team to support and develop research projects.
  • Aid improvement in clinical standards within the acute care setting to meet the Clinical Governance agenda within the Trust.
  • Participate in the development of evidence based clinical guidelines.
  • Participate in clinical audit.
