Job Summary

As a First Contact Mental Health Practitioner, you'll be part of a vibrant team delivering effective support to Primary Care Networks (PCNs) in providing high-quality mental health assistance within primary care, embedded in GP practices. Your role involves triage, assessment, and expertise in networking to facilitate access to various mental health services, ensuring each individual receives optimal care at the right time and place by the right professional.

Main Duties of the Job

  • Perform clinical responsibilities independently as a Mental Health practitioner in a GP setting, offering comprehensive triage services (including risk assessment) directly to individuals aged 16 & above, without initial GP consultations. Appointments typically last 30 minutes and are conducted through face-to-face meetings, phone calls, and home visits where necessary.
  • Conduct diagnoses within your practice scope and develop suitable management plans based on evidence-based practices and NICE guidelines.
  • Provide short-term risk management interventions within primary care, collaborating with patients and their support system to create risk management plans, which may encompass safety planning and self-harm management.
  • Recognize situations where risk cannot be managed effectively within primary care, and make timely referrals to specialist mental health services while maintaining patient safety and ensuring close coordination with these services.
  • Facilitate referrals to diverse services in primary and secondary care mental health, the third sector, and health and social care services.

Job Description

Job Responsibilities

Experience

Essential

  • Leadership qualification, training, or experience
  • Experience in managing an SMI register
  • Degree or equivalent qualification in evidence-based psychological therapies relevant to Pathway

  • Non-Medical Prescriber or readiness to pursue

  • Supporting staff in their daily clinical service provision
