Do you have a special interest or experience in frailty? As Anticipatory Care Advanced Clinical Practitioner within our Place-Based teams, you'll take the lead to develop a proactive frailty offer in line with national and local agendas, making a difference to the lives of our service users through preventative care.


As this is brand new role within the Trust, you'll have the unique opportunity to shape and tailor the role over time as you, and your work, develops.


Joining our Trust at this time will provide you with the opportunity and ability to shape our service aiming for the sustainability of proactive high standards of care for the Coventry population.

The successful applicant will be recognised for their high level of clinical skills, their wide range of experience, and will be a valued member of the clinical and leadership teams.


To be an autonomous practitioner and to act as a role model and a leader within the Community Health and Wellbeing Directorate (CHWB) at Coventry and Warwickshire partnership Trust.

The Post holder will be able to represent CWPT across the Integrated Care System as a respected contributor and influencer of projects and programmes such as the Anticipatory Care Project as part of the Ageing Well Programme.


The Post holder will be expert in advocating for a personalised care planning approach and achieving the best outcomes for their patients and will be able to motivate teams to succeed in achieving this.

The post holder will be able to recognise, assess, diagnose, manage, and review patients who are frail, have comorbidities, and the acute exacerbations of such long term conditions.


Part of the role of the Frailty Lead is to continuously work towards improving standards of care and benchmarking against National Standards and Agendas.

To enable this, the post holder will be able to collaborate and lead on developments in clinical practice for adults within their Place Based Team and across the Service.
