Cancer Support Worker, Haematology

Work with the haematology clinical nurse specialists to support patients undergoing a haematological diagnostic pathway, patients with a cancer diagnosis and patients living with and beyond cancer.

Work closely with clinicians, specialist nurses, admin staff and all members of the Multi-Disciplinary Team to ensure that patients are supported throughout the cancer journey.

Support the delivery of personalised care delivering Health Needs Assessments. Support Single Point of Contact phone line.

Support patients and carers to access appropriate cancer information and support. Effectively triage concerns individuals and carers affected by cancer may have. Work with the Cancer Nurse specialists to manage coordination of results and appointments. Communicate and support the delivery of Macmillan information to patients and carers. To maintain up to date knowledge of all targets and processes relating to the patient pathway process. Support and coordinate the EHNA tracking, ensuring the follow of needs identified. Support the delivery of the Recovery Package in line with the Cancer Alliance. Support and participate in audit processes and service development such as patient satisfaction surveys.

To work with and cross cover single point of contact and re-entry contact number. Work with the Cancer Nurse specialists to manage coordination of results and appointments. To maintain up to date knowledge of all targets and processes relating to the patient pathway process. Face to face and telephone communication and engagement with cancer patients and families. Ability to deal with level 1 distress and recognise and escalate concerns regarding psychological wellbeing and management. Demonstrate support and communication with patients. Involvement in the planning / organisation and delivery of health and wellbeing events and supported management events. Support and coordinate the EHNA tracking, ensuring the follow of needs identified.

Support the delivery of the Recovery Package in line with the Cancer Alliance and to work under the guidance and supervision of the CNS.

To support the department and organisation by carrying out any other duties that reasonably fit within the broad scope of a job of this grade and type of work.
