Job summary

An exciting opportunity has arisen to join our established and highly respected Critical Care Outreach Team. The team delivers a 24/7 service, cross site at Stoke Mandeville and Wycombe General Hospitals.

We are looking for a highly motivated, innovative and expert critical care practitioner to deliver patient-focused care to acutely ill, deteriorating and critically ill patients across the trust. The successful candidate will need to currently working at an Advanced Practitioner level and display excellent communication, interpersonal and leadership skills, to be able to respond to rapidly changing, high pressure situations and priorities .

You will be able to work autonomously and as part of a team and have excellent critical thinking and decision making skills. You will be instrumental in the early recognition of deterioration and deliver an effective response, liaising with the patient's own team. As an advanced practitioner you will be expected to fulfil the 4 pillars of advanced practice inclusive of clinical practice, leadership, education and research.

Main duties of the job

To provide expert advice, support and direction to ensure holistic patient care for acutely ill and deteriorating ward patients in conjunction with the multidisciplinary team over the 24-hour period.

Work closely with the Intensive Care Nursing and Medical Teams to facilitate early and timely admission to critical care and safe discharge from critical care

To provide the infrastructure that addresses the knowledge and skills deficits of ward teams in delivering acute care skills to patients.

Support the Implementation of national guidelines or evidence-based practice to improve patient care.

Provide leadership support for CCOT inclusive of deputizing for the Nurse Consultant for Critical Care and Outreach as required.

The Advanced Clinical Practitioner will provide highly specialized care within the hospital setting, to those patients with chronic and acute conditions using advanced clinical knowledge that is evidence based.

The Advanced Practitioner will function independently or within a clinical team and will demonstrate competence complementary to the MDT role in the delivery of health care.

Provide expert advice and to ensure the needs of the patient are met by leading, challenging and changing practice within the clinical setting.
