Purpose


Our Dual Diagnosis (High Intensity User) Caseworker will work alongside key local services to provide community outreach and brief interventions to individuals who have been identified as needing additional support due to ongoing unmet needs.

Individuals accessing the service will have a range of concurrent needs relating to mental health, substance misuse, and other complex needs, requiring additional support from Oxford University Hospitals (OUH) services.

The service will support individuals by identifying patients' needs, supporting them to make steps towards achieving their goals, and assisting them with accessing alternative, longer-term support effectively.

Building bridges for clients between hospital and community services and supporting a seamless step down into the community, will be key aims for this role.


Elmore Community Services are a leading expert in Oxfordshire, working with individuals who have complex needs and mental ill health who have fallen through the gaps of existing services.

Our assertive outreach model and supportive team approach help people improve their lives and integrate into the community.

Hours, Salary and Location

  • Salary £27,055.50 FTE per annum
  • Full & Parttime hours are available for this role (hours usually worked Mon-Fri between 9 to 5, although caseworkers may need to support clients outside of office hours)


Location
  • Head Office, Cowley, Oxford OX4 3UT.
  • 25 days (FTE) annual leave, rising by one day each year of service to a maximum of 30 days (FTE).
  • Pension, with employer's contribution rising to 5% on completion of probationary period.

Key Responsibilities

Provide a high-quality support service to clients:

  • To manage a caseload of allocated clients and liaise with the partner organisations to identify which individuals will benefit from a brief intervention.
  • To provide creative and flexible brief interventions to enable these individuals to develop selfmanagement strategies and produce plans to effectively manage their mental health and other needs and access suitable support in the community.
  • To act as an advocate for clients to ensure that their voices are heard.
  • To actively manage and update clients' risk assessments in line with Elmore's risk assessment policy and procedure and manage any safeguarding concerns effectively.
  • To maintain a wide knowledge of external multiagency services to facilitate and support client access to a range of agencies.
  • Develop and maintain a good partnership with other services by regularly attending and participating in team meetings and other meetings as required.

Contribute to the high performance of the wider Elmore team:

  • To work within the team approach and to be a supportive and effective part of the team.
  • To attend and participate in team meetings, planning meetings and policy reviews.
  • To work as part of a multiagency team, liaise with partner services, and provide data regularly to ensure accountability and evidence client engagement and activity.
  • To maintain clear and accurate records and complete relevant monitoring relating to the clients that you are supporting to fulfil contractual needs.
  • To actively promote Elmore to stakeholders and represent Elmore at external networking meetings and multiagency meetings regarding clients.
