We have an exciting opportunity to recruit a full time Specialist Mental Health Practitioner to work within Purbeck PCN.

The successful applicant will work across Swanage Medical Centre, Corfe Castle Surgery, Bere Regis Surgery, Sandford Surgery, Wareham Surgery and Wellbridge Practice.


These exciting roles are for those who are proactive and passionate about innovation and providing high-quality care to people who have been identified as in need of mental health support in a primary care setting.

The practitioner will access and link with secondary mental health services and help provide a "bridge" between primary care and secondary mental health providers.

The practitioners will be embedded as part of the Primary Care Network working across the practices which are part of the network.

They will work alongside the multi-disciplinary team and be closely linked to the Community Mental Health Team and Steps to Wellbeing Service (STEPS).

The Practitioner will provide triage, assessment, brief interventions and lead on development and implementation of specialist mental health training in the practice.


This new role will work collaboratively between Primary Care and Secondary Mental Health Services as part of an Integrated Model of Care.


The intention is to focus services on local communities and local GP practices to help rebuild and reconnect healthcare teams across the communities they serve.

This practitioner role will provide targeted care according to the needs of the population by supporting the PCN clinical strategy.


This role will work across an identified Primary Care Network to provide early assessment, treatment, and improved access to appropriate care pathways and provide evidence-based interventions for people with mental illness.


The practitioner will facilitate onward access to other secondary care-based services, third sector services and link in with existing support services via the Primary Care Network.
