About us:

Haslington Surgery is a two Partner practice, located in a small village in Cheshire. We have salaried GP's, four Nurses, one GPA and a Care Coordinator. This is supported by a Practice Manager and Senior Receptionist working with a small administration and reception team.

We form part of the local SMASH PCN group of Surgeries, working together to focus on local patient care and to deliver local services.


Together we are a friendly team who work closely and meet on a regular basis to promote continuous learning and professional development for all.

We are looking at expanding our team with a second Care Coordinator.

We have a flexible approach and will consider flexibility to hours worked during set working days.


We are supportive of professional development and pride ourselves supporting peer support for these new roles whilst working in a supportive and friendly environment.


Job Overview:

An exciting opportunity has arisen to work within Haslington Surgery. We are seeking a motivated, innovative Care Coordinator, with recent experience of this role, in a Primary Care setting. You will work closely with the clinical and administration teams to help and support the ongoing patient case management. This will involve linking Haslington Surgery with a range of Community Health Services.

You will support the Haslington Surgery team to deliver effective, co-ordinated care for vulnerable and frail adults, particularly those at high risk of a hospital emergency admission, emergency department (ED) attendances or out of hours care.

Your role will involve working proactively, driving service delivery.

This is a new and exciting role for you to develop and strive to support the needs of the Surgery going forward.

We are looking for someone who is hardworking, organised with great interpersonal skills who will be committed to delivering the highest quality of care for our Surgery.

  • Experience in a patient focused environment
  • Evidence of experience in wide range of administrative systems and software programmes
  • Experience of planning and organising complex meetings/agendas
  • Evidence of working with IT systems, in particular emis and docman.
  • Evidence of ability to support collation and analysis of data
  • Excellent verbal and written skills
  • Ability to provide and receive complex information
  • Excellent interpersonal skills
  • Excellent team player

Salary:
Is negotiable

- based on experience in a Care Coordinator role.

Working hours:18 hrs a week

- covering two days, Thursday and Friday, times negotiable.
