About the Role:


Reepham and Aylsham Medical Practice has a vacancy for a full-time care co-ordinator, with enthusiasm and commitment, to join our team to help us to provide exceptional customer care to our patient population.


Main duties of the job:

  • The postholder will, at all times, act within their professional boundaries, working alongside the general practice team for the benefit of patients.
  • They will provide and maintain a high standard of care, supporting the coordination and delivery of health care for a caseload of patients.
  • They will work closely, in partnership with all staff both internal and external to the practice, to develop the wider multidisciplinary team. This includes building relationships with other community providers and health services.
  • The care coordinator responsibilities include, but are not limited to, the following:
  • Managing all processes relating to multi-disciplinary team activities, such as coordinating meetings, administration and recording of all decisions and actions as appropriate, and maintaining the database register
  • Management of all processes relating to the PCN e.g., projects, communication, targets, IT (Ardens Manager & Care Navigator templates), staff HR, and meetings
  • Management of all patient services relating to job role
  • Preparing agendas for and chairing meetings relevant to team/job role
  • Managing all zero tolerance communications and actions with/to patients and external agencies as appropriate e.g., removal from patient list
  • Administration and reporting on IIF targets tracking & sharing with wider team all aspects of targets set
  • Maintenance of processes, coordination of practice response and liaison with PCN as required
  • Management of the processes relating to all significant events, complaints, concerns, and compliments, including full and proper investigation, response, escalation, recording and sharing as appropriate
  • Full administrative support for all safeguarding processes including (but not limited to):
  • Communication with all involved parties such as the safeguarding team and MASH
  • Clinical record data entry
  • Clinical System reporting
  • Contribution to policy, protocol, and template developments
  • Attend meetings relating to this role

All staff are required to:

Initiate contact with, and respond to requests from, patients, team members and external agencies, as appropriate to their job role

Maintain a clean, tidy, and effective working area

Contribute to public health campaigns (e.g. 'flu clinics) through advice or direct care

Support new members of staff on an as required basis

Any additional work reasonably request by GP partners or senior management

  • Experience of working with healthcare professionals and or previous experience in the NHS or social care or relevant field.
  • An appreciation of the nature of GPs and general practice.
  • An appreciation of the nature of primary care prescribing.
  • An appreciation of the new NHS landscape including the relationship between individual providers, PCNs and commissioners
  • Experience and an awareness of common acute and longterm conditions that are likely to be seen in general practice
  • Ability to follow legal, ethical, professional and organisational policies, procedures and codes of conduct
  • Excellent interpersonal, influencing and negotiation skills organisation skills with the ability to constructively challenge the view and practices of managers and clinicians
  • Ability to work under pressure and to meet deadlines
  • Good IT skills
  • Ability to maintain confidentiality
  • Ability to use own initiative, discretion and sensitivity

About Us:


We are a friendly, busy, rural, dual-site practice serving approximately 9,100 patients, in the catchment areas of Reepham and Aylsham in Norfolk.


As a GP surgery, we treat all minor illness and medical conditions, referring patients to hospitals and other medical services for urgent and specialist treatment.

We focus on the health of the whole person, combining physical, psychological and social aspects of care and have a large clinical and administrative team who support patients and staff to enable us to provide this care.


It is important that anybody that joins the team can work at pace and understand priorities and need to provide excellent care with a friendly, caring attitude.
