Virtual Ward is coming to Norfolk Integrated Care System with its main aim of keeping patients at home during acute illness and therefore avoiding unnecessary hospital admission.

This is a technological assisted approach with remote monitoring capabilities with a focus on patients with respiratory disease, frailty and heart failure.


This is a face to face model (also referred to as 'hospital at home') therefore a multidisciplinary approach is required.


As the Band 4 Registered Nursing Associate you will be instrumental in on-boarding our patients onto the Virtual Ward across Norwich.

Your role will involve performing an initial face to face assessment, undertaking investigations such as blood tests and observations/NEWS scoring, setting up the remote monitoring system and sharing information with each patient about the Virtual Ward process, contact details and what to do if they require help and support.


You will be part of a wider clinical team led by an Advanced Clinical Practitioner and together you will manage the Virtual Ward patients on the caseload by providing clinical face to face care, remote observations and contact via telephone.


A full training package will be offered to you on commencement of your employment as we aim to be innovative and fast thinking with our approach to caring for people at home who would otherwise be in hospital.


  • To provide and monitor safe, person centred care to patients admitted to Virtual Ward under the direction of a Registered Nurse, without direct supervision in line with an agreed plan of care.
  • To support the registered nurse in the contribution to ongoing assessment, treatment and care planning for patients under the care of Virtual Ward. To promote health and prevent ill health in patients and service users and contribute to integrated care
  • To monitor the condition and health needs of patients in partnership with colleagues, families and carers, referring to others for reassessment, when required
  • To actively contribute to an effective learning environment and support others in their learning
  • To adhere to the Nursing and Midwifery Code of Conduct for Nurses, Midwives and Nursing Associates and work within their scope of practice following the professional standards of practice and behaviours for nurses, midwives and nursing associates and the standards of proficiency.
  • This is a 7 day a week service and some weekend working will be expected. Hours of service delivery, 8am8pm

Provision and monitoring of assessment, treatment and care:

  • Provide holistic, compassionate, safe and effective interventions, care and support to patients, their families and service users in a range of care settings under the direction of a registered Nurse, without direct supervision and in line with an agreed plan of care.
  • Prepare for, carry out and monitor assessments and treatments with patients in line with competence level.
  • Develop understanding of all elements of the nursing process and be able to assist the Registered Nurse in the ongoing assessment, planning, management and evaluation of care.
