Carry out assessments risk/initial/comprehensive to inform treatment journeys.Research local resources to support referrals and work in partnership with other agencies to ensure that all clients' diverse needs are met.
Coordinate and work alongside Health care to monitor and manage client health and minimising risks of harmMotivate client to engage with support services identified in care plan and risk assessment.
Respects equality and diversity as defined in law, including any protected characteristics, and in consideration of best practice.
This role will pay particular attention to female service users and those from diverse communities, and we would therefore welcome applicants from these cohorts.
