Full-time. 11-month fixed term contract - with a view to extend. 4 roles available based across Kettering, Corby, East Northants and Wellingborough.

About us:


Support North Northants (SNN) is a collaborative service model, led by the voluntary sector, that can support people for a post-Covid world where people can have access to integrated and preventative services.

This will help to reduce pressures on statutory services and develop higher levels of community resilience.

We are putting transformation in practice and embarking on an exciting new way of working between partners.

Led by the North Northants voluntary, community, and social enterprise (VCSE) sector, we are working with the Integrated Care Service (ICS) to transform how services are delivered.


About the role:


We are looking for 4 Support Workers (2 for Wellingborough & East Northants, and 2 for Kettering & Corby) who will join our team and become a part of changing the culture of collaborative working with colleagues employed across the voluntary, statutory and private sectors and ensure service users 'tell their story once'.


Main responsibilities:

  • Provide direct personcentred, outcomes focussed support for people in the form of key working.
  • Develop outcomes focussed support plans and provide practical support as required.
  • Remain the key support worker for individuals until all positive outcomes have been achieved.
  • Work with a range of partners as if it was one team.
  • Update and maintain records and monitoring, ensuring accurate data is kept.
  • Contribute to the continual improvement of service effectiveness.
  • Identify any need for advocacy to support for people as required.
  • Identify any safeguarding concerns and take appropriate actions.
  • Have local knowledge of services and how to mobilise these for service users, including local gaps to help inform our strategic thinking.
  • Identify risks and ensure appropriate action is taken to mitigate risks.
  • Raise the profile and awareness of Support North Northants

About you:

  • Understand the purpose and transformation of the ICS.
  • Work with confidence with all partners and in genuine collaboration.
  • Quickly build effective working relationships across all sectors
  • Build relationships with all members of the community.
  • Be comfortable working with issues across a full range of community needs.
  • Be flexible and adaptable to meet the needs of people in the local community.
  • Be able to inspire and motivate an integrated team.

Knowledge and Experience:

  • 12 years' experience of working as a Support Worker in a health and social care setting or similar field of work (ideally L3 H & SC Qualification or equivalent)
  • 12 years' experience of working with local communities and community development or in social care area of work
  • Experience of personcentred, holistic and outcomes focussed practice desired
  • Experience of supporting the most vulnerable and 'not giving up on people'

Skills and attributes:

  • Ability to work with multiagency colleagues
  • The ability to think and plan, balancing needs and constraints
  • Ability to work effectively under pressure
  • Explores and identifies the range of risks in situations
  • Effective communication and keep the flow of information going to allow for quick resolution of issues

  • Must have full driver's license and access to a vehicle
