Job Duties:

The Adult Disability Team has an exciting opportunity for an Advanced Practitioner to work in the West Review Team.

You will be a SWE registered Social Worker or Occupational Therapist and preferably either an AMHP, BIA or Practice Educator.


The role will require you to lead on peer development and practice, support and supervise staff to develop their skills and knowledge in line with related legislation including The Care Act and Hertfordshire's Connected Lives principles to contribute to the ongoing development of the service.


You will work collaboratively with social workers, occupational therapists, and learning disability nurses within the care team to ensure a multi-disciplinary approach to the care of the people we serve.


You will be responsible for delivering high quality statutory social care including safeguarding, assessment, carrying out reviews, supervision and care management functions as well as developing innovative, creative community solutions.

You will be pivotal in developing links with the local community through undertaking multi-agency work with partner organisations, providers, professionals, individuals, families and carers.

The ability to converse at ease with members of the public and provide advice in accurate spoken English is essential for the post (for those whose language is a signed language the provision of a sign language interpreter who speaks English to the necessary standard of fluency will be required).
