We are looking for forward thinking, passionate staff who can take a positive risk management approach to support the frail population to remain at home when acutely unwell, within our Hospital at Home service in West Hertfordshire.


The Hospital at Home model has been developed with our system partners West Hertfordshire Teaching Hospital Trust and will blend digital monitoring and face to face care to facilitate acutely unwell patients out of hospital to home quicker or by preventing hospital admissions.


This is a new service which will expand upon of our existing virtual hospital models within our trust and will be implemented in line with National Guidance.


The team will be led by a Frailty Nurse Consultant and the service delivered by Advanced Clinical Practitioners, Enhanced Clinical Practitioners, senior nurses and therapists and support workers.

The Hospital at Home service will sit within the Unplanned Care Business Unit and will align with CLCH's Urgent Community Response service and cover the four localities in West Hertfordshire:
Dacorum, Watford and Three Rivers, Hertsmere and St Albans and Harpenden.


System working with our acute hospitals will be a key aspect to the delivery of this service which will generate the opportunity for you to develop skills in system working and service development.

  • To provide a high standard Occupational Therapy/ Physiotherapy support to all patients referred into the Frailty Hospital at Home service in West Hertfordshire.
  • To be an active member of the clinical team working within standard operating procedures and pathways to manage your own workload independently but with reference to a supervisor within the clinical team.
  • To support the team by assessing patients and implementing programmes of care ensuring that you follow professional standards, promote best practice and work within your sphere of competence and knowledge.
  • To undertake all aspects of clinical duties as an autonomous practitioner including assessing, treating and managing own caseload of patients and maintaining professional documentation.
  • Build and maintain relationships with other local services to facilitate appropriate referrals and high quality healthcare.
