Hospital at Home are looking fora motivated, enthusiastic Staff Nurse to join our newly developing service and multi-disciplinary team.
Hospital at Home is a new service alongside virtual wards specialities within Older Persons Services. Our aim is to provide excellent person-centred holistic care to our service users and their families.
The role will include assessments and implementation of plans of care to patients whom conditions and treatment can be met within their own home or within the community settings safety to prevent them from remaining within the acute settings when deemed appropriate.
Use of a car for transportation is essential.

This role requires current NMC registration
Based at: Christchurch, although traveling is required on a daily basis throughout the BCP locality

- Use of own care for transportation, with a valid Uk driving license
- Demonstrable clinical experience
- Experience of caring for patients with acute and long term conditions for older people
- Experience in providing end of life care
- Ability to demonstrate fundamental nursing knowledge and practice in the care of the older person
- Demonstrate understanding of own role and professional expectations and accountability in practice in line with NMC Code
- Awareness of role in relation to patient experience, quality and safety agenda's that support Governance Frameworks
- NMC registered Nurse
- Advanced Diploma/ Degree in adult health nursing studies
- Teaching qualification
- Post Reg course related to care of the Elderly
- IT skills
- Literacy and Numeracy
- Demonstrable ability in management of vital signs monitoring and escalation of the deteriorating patien
- Non-IV medication administration
- Venepuncture and cannulation
- ECG recording
- Male Catheterisation
- Awareness of audit in practice
- IV medication administration including blood products
