This is an exciting opportunity to join an NHS team providing specialist mental health support as a Social Worker in our Partnership organisation that promotes the standards for Employers of Social Work and subscribe to the Department of Health's Social Work for Better Mental Health programme, and the Think Ahead initiative.

You will play a key part in providing social care service and exemplar social work practice within the Forensic Mental Health Team ensuring that excellence of service is provided across the department. You will contribute to change management, concluding ECC's transformation and ensuring it is configured for continuous improvement adhering to the Key Performance Indicators as provided by ECC under the Section 75 partnership agreement.

You will be working closely with the social workers within the team, and the wider MDT within Brockfield House which is a medium/low secure unit. The role will involve working with the patients both male and female across the seven wards, assisting with debts, benefits, and any other practical issues they may have to ensure that they are equipped for discharge and integration back into the community.

You will be working closely with other agencies and Community teams. You will be expected to provide reports for the Mental Health Review Tribunal and liaise with external agencies such as the MoJ and MAPPA.

Accountabilities

Assess complex social care needs and provide support to vulnerable adults, which enable them to live healthily and independently. Develop support plans that gain commitment to solutions that drive the care required for each individual's needs. Accountable for working within the statutory legal framework, policy and guidance and determining when the threshold for statutory intervention has been reached · Work collaboratively building effective relationships with internal and external partnerships and stakeholders, and the voluntary sector to assist children and vulnerable people in gaining support for inclusivity from within the community. · Accountable for conducting assessments and plan responses to safeguard a child or vulnerable adult include intervention as required. Develop a client centric approach which enables early intervention and prevention and increases independence. Pro-actively influences in a range of planning, decision making and case discussions inter-agency. Work with families, communities and individuals to help people age well supporting the Council's wider strategic aims. About You To apply, you should be a qualified social worker with current Social Work England registration. You'll bring significant experience of social work, modelling good practice and providing advice and guidance in complex situations. As, as a subject matter expert in relevant professional practice, you should have a comprehensive understanding of current legislation, regulations, and guidance. With experience of chairing meetings involving clients, family members, carers, and other multi-agency professionals, we're looking for your ability to develop strong relationships with partner agencies and the local community to provide a supportive framework for vulnerable adults. You'll also bring: Evidence of continuing professional development and capability of practice in accordance with current professional standards. An in-depth understanding of the financial implications of commissioned care packages. The ability to make clear decisions, to highlight and limit potential risks associated with intervention. This role involves travelling around Essex and beyond. A driving licence and car - or the ability to meet the mobility requirements of the role through other means - is essential.
