In-reach Co-ordinator

Here at Nottinghamshire healthcare we looking for a motivated and passionate individual , to work within our In reach co-ordinator role at Highbury Hospital. This role will be based between Crisis home treatment team and inpatient acute wards and is part of the Urgent care team structure.
It is a role that looks at supporting flow and working closely with the wider MDT in order to facilitate an early discharge plan where appropriate for service users. The post holder will be able to make safe and effective plans alongside Crisis teams /LMHT's in order to ensure the safe transition of our patients back into their own homes
The post holder will manage two In reach practitioners, and the post holder will be responsible for management supervision and supporting the team with creating safe and effective discharges from inpatient services.
To work alongside the Team Leader and Clinical Leads in coordinating the team in the delivery of high-quality multidisciplinary care.The post holder will ensure patients receive appropriate intervention and treatment with minimal avoidable delays, whilst improving the quality and experience of care.
To be responsible for completing specialist mental health assessments and developing home treatment plans as an autonomous practitioner and to monitor interventions, ensuring compliance in accordance with Trust Policy.
To demonstrate active clinical leadership skills within the team, providing education and supervision to junior staff members and other professionals.
The post holder will be able to demonstrate professional skills and competence in leadership, care planning/co-ordination, care and treatment of those with mental health conditions. The aim is to drive system wide change and enable more people to live in the community, with the right support, more people to be cared for in inpatient settings closest to the place they live and to ensure timely and safe discharge from inpatient care.To maintain professional standards, ensuring adherence to all relevant policies and procedures to ensure the delivery of high-quality care. To work alongside the Team Leader and Clinical Leads in coordinating the team in the delivery of high-quality multidisciplinary care.

In collaboration with the team leader monitor and lead improvements to standards of care through, supervision of practice, clinical audit, evidenced based practice, teaching, and supporting professional colleagues and the provision of skilled professional leadership.

- Registration with NMC or equivalent
- A relevant health care qualification at master's degree level or relevant clinical experience.
- Engagement and intervention-based skills
- Evidence of sound care planning skills
- Teaching and assessing training
- Evidence of formulating and evaluating positive behaviours support plans
- Evidence of post-registration training
- Clinical supervision course
- Ability to travel across a wide geographical area.
- A full UK driving licence (with access to a vehicle for work)
- Ability to work in both the community and inpatient setting
- Experience of managing a team
- Experience of working within Crisis team or inpatient acute ward
- Experience of individual client work within an AMH setting
- Experience of engaging effectively with clients/carers and other professionals
- Experience of attending and contributing to multi-agency meetings
- Substantial experience of MDT working
- Acute mental health experience
- Substantial and considerable experience in assessing people with an acute mental health disorder
- Community mental health experience
- Crisis team experience
- Relevant clinical courses
