Job Introduction

As a Care Co-ordinator you'll join a team who are focused on delivering impeccable customer service to our staff and service users to really make our service users and staff feel the difference.

HCRG Care Group provides community services across the West Lancashire area, with the Single Point of Access Team being an integral part of this service.

As a Care Co-ordinator, you will provide excellent customer service to patients, GPs, etc who access this community hub.


Alongside this role, you will complete a Level 2 or 3 (depending on experience) Customer Service Apprenticeship, provider by our Ofsted rated 'Outstanding' internal training provider, The Learning Enterprise.

Main Responsibilities


Supporting clinical teams by booking and scheduling routine clinic activity based on rules and protocols and signposting other activity to the relevant services within required timescales.

The CCH is operational 8am - 8pm, 7 days a week

  • Have 5 GCSE's grade A-C including Maths and English or equivalent.
  • Be able to plans and manage their own workload on a daily basis.
  • Have confidence using IT systems and be able to demonstrate working knowledge of Microsoft Office packages i.e. Outlook, Word, Excel.
  • Have a great attitude to work
  • Always be looking to develop and learn
