Team Leader Responsibilities

Main purpose of Job:

  • To be the first point of contact for the homes
  • To take active part in local systems reporting and approvals
  • To ensure that young people's case files are monitored
  • To check that care plans, behaviour support plans, risk assessments, and are all in place and are reviewed regularly
  • Responsible for on call
  • To check progress reports monthly within a target period and filter to the line manager once this is complete, once the manager has checked this, ensure that the key workers forward on to the relevant professionals
  • To ensure that the progress reports are forwarded to the social workers or other key stakeholder
  • To ensure that housekeeping checks are being completed
  • To perform medication checks to ensure that this is completed properly with the five rights
  • To check appointments for children and young people with Outreach workers other stakeholder
  • To monitor cleanliness of the homes
  • To monitor maintenance issues and follow up on repairs needed or completed
  • To be involved with completing staff supervision and spot checks
  • To be involved with the oncall systems
  • To ensure that key work sessions are being completed on a regular weekly basis
  • To ensure that all children and young people's meetings and appointments are being met
  • Attend reviews when key workers are off on annual leave
  • All seniors must ensure that they feedback to the line manager and governance teams
  • To be responsible for Inventories within the homes.

Additional Support Worker Responsibilities

  • To ensure consistent delivery of highquality services to service users, partner organisations and staff alike in accordance with the policy and procedures of the organisation.
  • To undertake direct work with young people living in our 24hour accommodation which aims to enable them to gain the confidence and skills to live independently as well as supporting them practically and emotionally in their daytoday lives.
  • To ensure that the children and young people are effectively inducted into the homes and ensure that the young people have a good experience and are shown around the home. Given a copy of the young people's guide, shown all fire exits, introduced to the key worker and other children and young people at the home and to work through their occupancy agreement and rules of the home and sign all relevant documentation
  • To form caring, respectful, trusting, and professional relationships with all of our allocated children and young people.
  • To contribute to the holistic assessment of the individual young person's current needs including writing detailed and regularly reviewed personcentered support plans that include the children and young person's views, wishes and feelings for their ongoing support.
  • To compile and work through with the children and young people and other stakeholders, comprehensive individual risk assessments to support children and young people to identify and manage their potential risks and to withdraw from potentially harmful relationships, and support the team with understanding triggers, push and pull factors, and to identify any other risks such as CSE, CCE, Gang culture, FGM, the list is non exhaustive.
  • To write comprehensive individual daily reports, and handovers, incident forms, key work sessions, daily health and safety checks and all other relevant forms utilising our Clear Care software platform.
  • To assist young people in completing actions as identified in their support plan/risk assessment (i.e., help with claiming benefits, dealing with tenancy issues, finding employment training or education, registering with GP, Dentist, optician etc.). non exhaustive list
  • To provide emotional and practical support to young people in their daily lives and any issues which may arise.
  • To support young people to meet with required cultural communities or places of worship.
  • To work through the 'Transition into Independence' programme with children and young people where required.
  • To deliver services within a solutionfocused brief therapy approach and act as a positive role model for children and young people.
  • To hold regular planned key work sessions with children and young people and other stakeholders either in the home or out in the community
  • To assist young people in making timely benefits claims and to monitor/intervene in ongoing claims as needed.

Service Delivery

  • To deliver services in accordance with the organisation's policies and procedures always.
  • To encourage and support children and young people's participation and involvement in service delivery and design.
  • To ensure case records and reports are completed clearly and accurately and comply with the relevant quality standards.
