We are looking for an experienced, ambitious and forward-thinking individual who has experience in hospital pharmacy to help shape and drive patient care and innovation across mental health services.

You will support the review of clinical pharmacy service provision across hospital and community services (across Stafford and Shropshire) ensuring that clinical service provision is optimised and that service users have the opportunity to discuss their medication with a member of the Pharmacy & Medicines Optimisation Team.

You will be highly motivated, confident and experienced with excellent communication skills and a postgraduate qualifications or experience in mental health therapeutics.

It is expected that the successful applicant will be an experienced clinical pharmacist with a substantial track record of providing or supporting service transformation, excellent time management and communicative skills.

Main duties of the job

Clinical pharmacy service provision across hospital and community services ensuring that clinical service provision is optimised.

To support and develop pharmacy systems to enhance the transfer of care for service users.

To work with local community transformation teams to identify new ways of working for pharmacists and pharmacy technicians in new community mental health models.

To develop a process that identifies patients with repeated admissions due to poor adherence to medicines and adverse drug reactions.

To assist with partnership working with community, primary care networks and hospital pharmacy to help utilise skill mix, improve patient outcomes.

To be responsible for providing and improving quality assurance, risk management.

To scope the need for education and training for primary care network (PCN) pharmacy /GP/community pharmacists on mental health (MH) medicines use and treatment.

To deputise for the Deputy Director of Pharmacy (Mental Health) as required.

Operational leadership of the clinical pharmacy service within the community hospital settings.

Training and supervision of undergraduate, foundation year trainee and/ or post graduate pharmacists and trainee technicians.

Job responsibilities

Reengineering of mental health inpatient and community based services to ensure the optimal impact of pharmacist skill set is fully realised and that patients have access to a member of the pharmacy team to discuss medication related issues Work with the Deputy Director of Pharmacy & Medicines Optimisation - Mental Health to provide expert advice on medicines optimisation to the Shropshire, Telford & Wrekin Care Group advising on cost effective prescribing, national policy directives, service development, audit and risk mitigation. Manage band 8a pharmacists across the corporate team, to deliver clinical services and medicines optimisation, including audit and monitoring. Clinical and managerial supervision of band 8a pharmacists across mental health services. Assist with partnership working with community, hospital pharmacy and primary care networks to help utilise skill mix, improve patient outcomes, ensure better access to healthcare and help manage workload. Monitor, analyse, interpret and provide advice on medicines related data, clinical audit and incident reports ePACT data etc, and arrange for cascade to managers and staff as appropriate. Advise on cost effective prescribing and more efficient use of resources where appropriate, especially in relation to the Shropshire, Telford & Wrekin Care Group. To provide professional advice and support for the Shropshire, Telford & Wrekin Care Group. Provide written and verbal reports, accounts and feedback on any work undertaken in support of service delivery. Support services to fulfil the medicines optimisation requirements of the Care Quality Commission (CQC), including developing and reviewing standard operating procedures (SOPs), medicine guidelines / protocols and clinical guidelines, in conjunction with the Governance & Community Pharmacist. Responsibility for the development and review of operational guidelines for clinical pharmacy in mental health settings and working in conjunction with the Chief Technicians for Hospital and Community services to ensure that operation guidance includes technical service provision. Provides highly specialist pharmacy services relating to medicines-related care to patients, prescribers, clinicians, carers. Accountable for delivery of mental health clinical pharmacy services, ensuring compliance with medicines legislation. Person Specification
