An exciting new opportunity has arisen to join our friendly department as part of a Cross Site Medicines Optimisation Pharmacy Technician Team. These are new roles, which would suit an experienced solution focused Medicine Management Technician who is happy to work autonomously in a patient facing role.

The post holder will rotate between Community Mental Health Services and hospital services to provide support to the community mental health teams within Shropshire. You will work independently to provide a seamless pathway from secondary to primary care and participate in the discharge planning process.

As a member of the Pharmacy Team at MPFT you will ensure the safe, effective and economical use of medicines within the mental health sector, to provide support with Depot clinics and Clozapine service.

Main duties of the job

JOB PURPOSE

The post holder will provide expert advice and support on aspects of medicines optimisation to ensure the use of medicines is optimised and meets safety, statutory and governance requirements. The post holder will act as first point of contact for any medicines management issues for the community mental health teams. Work autonomously and be responsible for planning and delivering own workload and agreeing deadlines with Chief Technician. Provide professional advice and support on the safe prescribing, handling and administration of medicines. Rotate between hospital wards and community services. To perform medicines reconciliation on admission to hospital and on discharge as necessary. To provide medication counselling to promote self-management and independence with medication. To participate in discharge planning. To ensure a seamless pathway from in-patient to out-patient. To support development of a comprehensive medicines optimisation service to the wards and clinics. Support service development and be responsive to emerging problems escalating where necessary. The post holder will work to promote safe, legal, rational and economic use of medicines by all users of the pharmacy service.

Job responsibilities

Community Services

The post holder will support the development and delivery of the clinical pharmacy service provision across the Community Mental Health Service. This will involve obtaining accurate drug histories on admission and discharge by liaising with secondary care providers, documenting and referring findings where necessary.

Responsible for implementing and monitoring the Trusts clinical pharmacy standards as well as ensuring the clinical service is of high quality and patient focused. They will identify gaps and problem areas in the service and use appropriate methods including quality improvement to improve the service.

Co-ordinate the supply of Depots from polar speed.

Co-ordinate the supply of Clozapine for community patients.

Support the maintenance of the repeat prescriptions, ensuring they are in date and all necessary monitoring has taken place. Highlighted discrepancies in a timely manner to avoid unnecessary waste or dose inaccuracies.

Review community mental health patients to ensure physical monitoring has taken place.

Review clozapine patients to ensure essential serum monitoring has taken place (according to set monitoring data)

Participate in the counselling of patients in the community setting.

The post holder will support the ESCA referrals .

The post holder will participate in patient counselling sessions in both Clozapine and Depot clinics.

Clinical Role

The post holder will provide advice and counselling to patients /carers on medicines in line with departmental standards and SOPs.

Carry out medicines reconciliation on admission and on discharge, in accordance with departmental standards and SOPs.

Clozapine Dispensing

The post holder will lead on the co-ordination of the supply of clozapine to community patients. This will involve the entering and checking of blood results on the eCPMS system according to the strict procedures stipulated by the Manufacturers Product Licence and liaising with Haematology staff as necessary.

The post holder will be expected to liaise with external suppliers regarding clozapine prescriptions and transport services to ensure delivery, and follow up outstanding monitoring /queries.

The post holder will monitor patients to ensure serum level are taken and highlight issues to the appropriate clinician.

Ward Services

The post holder will complete medicine reconciliation on admission and discharge.

Provide patient counselling at ward level as necessary.

Identify patients due for discharge and provide counselling to improve compliance/ concordance aiming for a smooth transfer back to community services. Highlight and refer patients into the community team as appropriate.

Lead on the Discharge Medicines Service (DMS).

Prescription processing

The post holder may need to supervise the dispensing of medication to inpatients/outpatients.

Organise with doctors (or their secretaries) the supply of repeat prescriptions (clozapine/depot injections) for those patients whose prescriptions have expired.

Support the nominated dispensary in the role of accuracy checking technician (ACT).

Person Specification

Qualifications

Essential

BTEC and NVQ3 (or equivalent) in Pharmaceutical Science. Nationally recognised Accredited Checking Technician Certificate. Registered as a Pharmacy Technician with the General Pharmaceutical Council.

Desirable

Ability to produce reports using a variety of systems

Experience
