This is an exciting opportunity, due to internal promotion and service expansion for an experienced, registered professional to join a dynamic service, responsible for ensuring recovery focused care pathways for all forensic clients within specialised mental health care settings, within the NHS and the independent sectors.


The post holder must be adept at prioritising the activities that will provide the most efficient, effective and safe step down or discharge pathway and escalate any conditions that will prevent this from happening.

The post holder will be responsible for the development and monitoring of the assigned care pathway quality and improvement plans.

The post holder, in conjunction with the management team, will ensure all clinical effectiveness systems are embedded and sustainable (this will include audit and evidence-based care).

Oxleas offers a wide range of NHS healthcare services to people in community and secure environment settings.

Our services include community health care such as district nursing and speech and language therapy, care for people with learning disabilities and mental health care such as psychiatry, nursing and therapies.

  • Overview of the Post


To work as part of a dynamic team delivering a service to ensure effective, safe and compassionate, care pathways which support recovery are in place for all service users from the SLP in specialised mental health care settings, within the NHS and independent sectors.

The long-term aim is to reduce the number of SLP patients being admitted to non

  • London services. The team will maximise the use of inpatient capacity across London specialised mental health services ensuring value for money for patients.
  • Key Task and Responsibilities

Management responsibilities


Ensuring that patients' care pathways are monitored and those patients at heightened risk of delay transition through the service appropriately.

This includes attending (or participating in by videoconference, skype or telephone) Borough liaison and referral meetings.


Ensuring that service users ready for discharge are discharged or stepped down a level of security within the appropriate timeframe.


Providing summary reports to senior managers as required (these might include patient pathway reports, activity reports, bed occupancy data, or caseload data for instance).

Develop and promote positive relationships with housing providers, tenancy support teams and benefit agencies and others.

Deliver a service with the agreed budget ensuring maximum value in terms of clinical and cost effectiveness.


Where appropriate, and in conjunction with the relevant line manager, supporting recruitment to help ensure that practitioners with the appropriate knowledge, skills and competencies deliver services.

Deputise for the integrated Team Manager as and when required.

Analytical & Judgement

Managing complex cases and analysing data for the SLP

PH will need to interpret the data given in order to sit patients in the correct specialism.

Leadership


The post holder will be accountable to the Integrated Team Manager, The Head of Forensic Social Work and the Forensic Case Manager.

The post holder will be responsible for the supervision of junior staff as directed by the Senior Management Team.
