Job summary
Are you caring and compassionate? Do you have a passion for working with people with dementia? Do you think you could make a difference?We are looking for a new member of the dementia crisis team, providing care and therapeutic intervention to patients in a range of community settings under the guidance of professionally qualified members of the team.
Main duties of the job
The post holder should have a passion for assisting disabled and/or vulnerable adults to live at home with dignity and independence while supporting them by providing rehabilitative care.
The post holder would need to undertake a range of delegated clinical patient care duties in a range of settings, to maintain patient assist in the ongoing development and evaluation of the service

NVQ Level 2 in healthcare or equivalent
Qualification relating to caring for people living with dementia, or NVQ level 3
Experience
Proven experience working with vulnerable adults with complex and varying needs within a social or health care setting
Multi-disciplinary teamwork
Understanding of the need for enabling patients to achieve optimum independence
Experience of working with people with dementia
Experience of working in a community setting
Special knowledge / expertise
Demonstrate a good working knowledge of dementia and importance of person centred care/therapy
Awareness of support available for older people
