Are you the right person for the job?

  • Experience in supporting vulnerable adults in a personcentred way
  • Knowledge of strengthbased recovery models and approaches

What will your role look like?

  • Provide holistic onetoone social prescribing support for people aged 18 and over who are frequent presenters at GP surgeries
  • Entail actively contributing as a member of a wellestablished social prescribing team who supports the most vulnerable in society
Mileage is paid for all posts at NJC rates where applicable
