TEAM INFORMATION An exciting opportunity has arisen to join our Short Breaks Team. Our team support children and young people with disabilities and complex health needs to access their community, develop friendships and gain independence, along with overnight short breaks with an approved Foster Carer. The Short Breaks service is all about enabling young people to live their very best lives, exploring their potential and giving them the very best opportunity to have a meaningful and beneficial break. It's also about ensuring families and full-time carers are able to find the time for themselves, which can include doing their housework, shopping, or just having a rest As part of a young person's support network, you will be expected to build a positive relationship with them and their family.


The Short Breaks Service operates citywide and provides outcome focussed planned short breaks for disabled children and young people in order to promote growth, development and inclusion, as well as providing support for families and carers.


Team members work with a wide range of colleagues both internally and externally, to ensure the Local Authority meets its statutory duties under the Children's Act, as well as maintaining and forging business relationships with commissioning groups and other relevant stakeholders.

We are a Care Quality Commission (CQC) registered service, and we currently hold a GOOD rating.


Short Breaks Workers provide support and guidance to link workers and sitters and are responsible for reviewing ongoing work with disabled children and young people.

They also review care plans and risk assessments in partnership with a senior member of the team, to ensure that the support provided is person centred, outcome focused and encourages independence.

Liaison with other professionals is an important part of the role, ensuring that we have the most up to date information and knowledge of how best to support children and young people who use our services.


Some of our young people may require personal care, or support with their mobility needs for example, and it's important that this is managed with their dignity in mind.

Compassion, patience and understanding is a must.

There may also at times be challenging situations on link work sessions, as we do support some young people who can display behaviour that challenges, so the ability to think fast and react appropriately - and sensitively - is also essential.

Our more complex children and young people may require 2:1 support (based on their assessed needs).

Short Breaks Workers will be able to support the Linkers/Sitters with these situations, offering guidance and advice both during office hours and also when covering the service 'On Call' phone (on a weekend rota).

Short Breaks Workers are also required at times to provide direct support with children and young people where necessary.

ESSENTIAL REQUIREMENTS:

  • Experience of working with disabled children, young people and their families;
  • Experience of undertaking specific pieces of work relating to care plans and risk assessments;
  • Ability to prepare case records, reports and simple correspondence whilst ensuring confidentiality.
