My Horizon Care is looking to recruit an experienced Care Coordinator in Leeds.
We are looking for an experienced Domiciliary Senior Care Coordinator with a full UK Drivers license.


We seek to continue to offer the best of services to clients with diverse needs and helping them to live safely and independently in their own homes.


Applicants must have held previous positions as Care Coordinator for at least 2 years and experience as a Care Worker and able to adapt into a new environment.

Driving is essential due to the nature of the role.

Duties-

The Care Coordinator's duties will include:

  • Conducting client Initial assessment and writing up person centred care plan and risk assessments;
  • Accepting care packages and assigning care workers to service users in an appropriate manner;
  • Allocation of care workers to customer visits by using effective rostering, ensuring continuity of care and taking into account locations, skills required and customer preferences.
  • Recruiting and retaining care workers to support growth and expansion;
  • Managing care workers and conducting care worker supervisions on monthly basis;
  • Ensuring that service user and care worker files are complete to the expected standards;
  • Using electronic monitoring software, as well as conducting spot checks to monitor care workers' attendance and timeliness for visits;
  • Arranging care reviews and risk assessments for service users with the field supervisor;
  • Conducting telephone monitoring and service user visits and ensuring service quality;
  • Liaising with clients, local authorities and other stakeholders;
  • Representing the company during meetings with officials and service commissioners;
  • Attending training events and ensuring that skills and knowledge of industry best practice is kept uptodate;
  • Ensuring that our service complies with and exceeds CQC requirements;
  • Ensuring that the highest standards of care are maintained at all times.
  • Allocate all unassigned calls during oncall hours
    where visits cannot be assigned, undertake the visit
  • Undertake oncall duties as directed and record/report all oncall activity to the office.
  • Ensure that telephones are answered promptly and people are spoken to in a polite and respectful manner.
  • Maintain confidentiality at all times and carry out the Company's Confidentiality Policy.
  • Carry out the monitoring of the service in the field to ensure that it meets the quality targets of the Company, contractual requirements of the commissioners and report to registered manager outcomes and issues that are identified.
  • Review Care Plans, medication records and daily care notes recorded by Care workers.
  • Assist with staff induction and shadowing.
  • Assist the Manager in holding team meetings for Care Workers.
  • Maintain accurate records such as service users authorised hours, number of visits, communication with health professionals as well as office communication.


The position of Care Coordinator is an important role in the company and will require the post holder to ensure the smooth delivery of care and support to our clients.
