The successful candidate will be supported with their leadership team through dedicated supervision with highly experienced and motivated team managers, where learning opportunities will be supported and encouraged. From the multi-disciplinary team, we offer regular CPD sessions and candidates are encouraged to bring their own interests to the team to share and develop their specific interests in mental health.

Main duties of the job

The post holder will work with people providing support to service-users working in a discreet Care Group alongside Primary Care Networks in the South of Sheffield. You will promote positive outcomes by working with people in a proactive, practical sense encouraging service-users independence. You will work with your care team to ensure the service promotes healthy lifestyles, opportunities to enhance knowledge about mental health and provide encouragement to participate in care. You will liaise with other services to ensure a service-users journey remains as seamless as possible and report back to the MDT in a weekly meeting.

Detailed job description and main responsibilities

Acting as a Named Worker for a designated caseload to ensure service users receive a consistent approach and a named person to contact. This will involve building a working relationship and to promote an individual's ability to manage their own needs and independence. As a named worker you will work in collaboration with the Multidisciplinary team to support the overall care and treatment plan of the individual. Develop a rapport with service users within appropriate boundaries. To encourage service users to be fully involved and engaged with their agreed care plan and to access appropriate services. Implement agreed programmes of care collaboratively with service users and carers under the supervision of a named professional. Help to identify early signs of relapse by monitoring the service user's progress, level of functioning and mental state and alert the appropriate staff involved in their care. To take a role in evaluation of care provided To identify and support carer needs as required. To take a role in health promotion including physical health needs. Participate in and help run social activity groups.

Essential criteria

•NVQ Level 3 in Care or the equivalent/or be willing to work towards the qualification.
