Job Overview

The South Community Mental Health Service is undergoing a transformative journey, with additional resource allocation to provide ongoing intensive support to the population of Sheffield. This includes working towards a joint approach with Primary Care and the Voluntary Care Sector, reviewing the Community Mental Health Team (CMHT) Service Criteria, and working in discreet small teams within local communities to achieve a high-quality care standard.

Main Duties

The post holder will work with individuals, providing support to service-users, in a discreet Care Group alongside Primary Care Networks in the South of Sheffield. You will promote positive outcomes by working with people in a proactive, practical sense, encouraging service-users' independence. You will work with your care team to ensure the service promotes healthy lifestyles, opportunities to enhance knowledge about mental health, and provides encouragement to participate in care. You will liaise with other services to ensure a service-users' journey remains as seamless as possible and report back to the MDT in a weekly meeting.

Key Responsibilities
  • Acting as a Named Worker for a designated caseload to ensure service users receive a consistent approach and a named person to contact. This will involve building a working relationship and promoting an individual's ability to manage their own needs and independence.
  • Develop a rapport with service users within appropriate boundaries.
  • Encourage service users to be fully involved and engaged with their agreed care plan and access appropriate services.
  • Implement agreed programmes of care collaboratively with service users and carers under the supervision of a named professional.
  • Help identify early signs of relapse by monitoring the service user's progress, level of functioning, and mental state and alert the appropriate staff involved in their care.
  • Take a role in evaluation of care provided.
  • Identify and support carer needs as required.
  • Take a role in health promotion, including physical health needs.
  • Participate in and help run social activity groups.
