An opportunity for a Band 3 rehabilitation assistant has arisen within our Community Stroke Service. The service is in line with the national service model offering stroke patients rehabilitation at a range of intensities.


Within this role you will work as part of the interdisiplinary team assisting in the delivering of rehabilitation programmes for newly diagnosed stroke survivors to maximise their functional abilities.


You should have patient facing experience in a rehabilitation role, have good communication skills, be able to work independently and as part of a team.

You will need to be flexible and work in a wide range of settings across the community and manage your time effectively.


You will be supported by other clinicians within the service and there are very good opportunities for development and career progression.


The post holder will work with stroke survivors as part of a multidisciplinary team to facilitate their early discharge from hospital and enable them to achieve the maximum possible independence and support them to adjust to life post stroke.


The post holder will assist all disciplines in the service to deliver their rehabilitation programmes, undertaking specific skilled support work within clinic settings, the client's home or in the community.


The post holder may work independently without direct supervision at times, however they will be in regular liaison with designated therapists who will assess all patients and provide a rehabilitation programme to the post holder.

The post holder will promote patient independence whilst following a rehabilitation plan for rehabilitation.


An important element of the role is to ensure patients are empowered to make decisions about their care and are encouraged to exercise choice.
