We have a 5 year Strategy driven by our community, underpinned by 28 Promises to our community, that provides direction and stability in driving positive improvement across all age services.
We can't do all this alone, and to deliver the strategy the Trust needs to strengthen relationships with Primary Care, specifically in relation to promises 12, 13, 15 and 21.
We are looking for someone who can take a senior lead in delivering programmes of work with primary care, also working with our great care groups, and importantly maintaining our focus to nurture the power of communities by working with the VCSE and our community.
We are looking for someone who can make this happen, work with a degree of ambiguity at times, and develop joint programmes that match the scale of not just our ambitions, but importantly our communities ambitions.
The post holder will, under the leadership and management of the Deputy Director of Strategic Development, fulfil this role by taking a senior lead in delivering programmes of work relating to Primary Care.
- Building relationships with Primary Care;
- Joint project development;
- Improving system and processes across primary and secondary care to improve the coordination of care;
- Working with Care Groups to establish neighbourhood working with Primary Care;
- Sourcing funding for joint projects and developments.
