Main Responsibilities:

  • Collaborate on care planning with service users and their carers
  • Conduct risk assessments and record them according to policy
  • Coordinate shifts when required
  • Participate in Multi-disciplinary Team (MDT) meetings and create care plans with patients
  • Stay updated on evidence-based home treatment interventions and implement them effectively
  • Contribute to gatekeeping assessments for least restrictive treatment
  • Allocate shift coordination tasks
