Job summary
To provide support for clients presenting with complex behaviors that challenge, who receive services from the Tower Hamlets Community Learning Disabilities Service for Adults with Learning Disabilities, with a focus on providing a range of person centred assessments and interventions in line with a Positive Behavioural Support approach.
Main duties of the job
To work alongside clients, families, support workers and professionals to enable others to better understand and respond to a client's behaviours that are challenging in line with PBS principles and Positive & Proactive Care: reducing the need for restrictive interventions Interventions will include working across a variety of settings, including residential and day services and this will involve working with the multi-disciplinary team. The post-holder will be involved with research and evaluation of local service and will play a key role in delivering relevant training to increase the knowledge and skills of the appropriate stakeholders. Acting autonomously the postholder will manage a defined caseload of complex cases independently, evidencing problem solving and clinical reasoning skills in line with evidence based and client-centred principles. S/he will be involved in clinical development where appropriate. Job descriptionJob responsibilities
Patient Care
Ensure a high standard of clinical care for the clients. Play a key role in the assessment of clients who present with behaviours that challenge, undertaking both initial assessments and functional analyses. To provide a range of behavioural intervention strategies based on the functional analysis of a client's behaviours which are suitable for service users living with their families and/or support staff, this will include the development of Behaviour Support Plans. Undertake a significant clinical caseload working under supervision in collaboration with other agencies and health and social care professionals. To work with other members of the team on developing the knowledge and skill base around Positive Behaviour Support in the local borough this may be for family carers, support staff, and/or other professionals. This may include working alongside staff that providing direct support to clients to enable them to develop their skills to work effectively with individuals with behaviours that challenge. Communicate effectively with health and social care professionals and outside agencies regarding client care and progress. Communicate with other members of the Team regarding policies, service development and management plans. Along with other members of the community team, to be actively involved in delivering recommendations in line with the Transforming Care Programme and in implementing the local Inner North East London TCP Plan.Clinical
Work autonomously in a variety of work contexts, including work in the community which may at times involve working with clients with complex needs in their own home. This includes face to face sessions with individual clients, families and support workers.
Spend sustained amounts of time with service users who may be challenging such as aggressive and hostile, who may have poor communication skills and self-care and special physical and/or mental needs.
Adapt verbal and non-verbal communication to facilitate clients understanding.
Liaise appropriately with all agencies involved and keep them informed of relevant information and recommended guidelines necessary to provide high standards of care for clients, within the recommended limits of confidentiality.
Provide a clinical service to clients from a range of minority ethnic populations and provide an appropriate specialist person-centred programme of care taking into account language, religion, cultural beliefs and customs and adapt assessment, formulation and intervention as appropriate.
Accountable for one's own actions and to work within Trust and NHS code of practice and ethics, and Trust policies and procedures.
Responsible for offering specialised advice, guidance and support to staff in the joint community team for people with learning disabilities, to staff teams in statutory and voluntary organisations and family carers of people with learning disabilities.
Provide advice, guidance and support, both formally and informally, to staff, families and carers.
Responsible for developing and maintaining knowledge of local resources and developing working relationships with relevant statutory, voluntary and community groups and organisations.
Other relevant duties as discussed with the Lead Clinical PsychologistAdministration
Plan and prioritise own clinical workload and to work with multi-disciplinary colleagues around complex cases, providing this in a setting which is most appropriate to the client and/or service needs.
Responsible for recording, monitoring and reporting on clinical work and communicating clinical information to a variety of recipients ( service users, families, carers, other professionals, and statutory and voluntary organisations) orally, in writing and electronically, and as specified by the service. Also to maintain client records as an autonomous practitioner.
Management
In conjunction with the Lead Clinical Psychologist contribute to the development of a high quality challenging behavior pathway. Contribute, as a member of relevant working groups, to the development, implementation and evaluation of the Team. Implement Trust and local policies. Organise own day-to day activities, plan and prioritise own clinical and non-clinical workload. Identify, propose and lead changes to work practices and procedures in own area of work and join working groups leading on service wide and specialism developments as appropriate.Human Resources
