An exciting opportunity has arisen to support the continued development of Neighbourhood Working in Lincolnshire working as a Specialist Neighbourhood Practitioner.

This role will work in East Lindsey Neighbourhood in partnership with East Lindsey and Meridian Medical Primary Care Networks.


Neighbourhood Working is an integrated, collaborative approach which brings stakeholders together from across the health, care and community/voluntary system at local population level.

Neighbourhood working puts people's health and wellbeing needs at the centre of decision-making, overcoming barriers and promoting proactive support so people stay independent and close to home.


Specialist Neighbourhood Practitioners:

  • Provide clinical leadership to neighbourhood working ensuring clinical oversight of all people referred into neighbourhood teams.
  • Coordinate complex care and provide a vital link between primary secondary and specialist service.
  • Undertake comprehensive assessment of people with complex needs developing personalised plans to support proactive care close to home.


The post is offered as a 12 month fixed term contract or secondment offering an excellent opportunity to develop and build skills and relationships in a supportive and progress focused team.


You will be highly motivated and committed to empowering people to express what matters to them and striving to ensure people achieve their outcomes.
