Job summary

The @home service are seeking a Highly Specialist Occupational Therapist (Band 7 ) to fill a full-time position within the @home service.

We are continuously expanding our urgent community response in line with NHS England's Long-Term Plan to support London's ageing population and those with complex and urgent needs. Virtual wards is a focal point currently as well as providing a falls response to create greater community capacity. Collaborative partnerships with health and social care stakeholders ensure we place our patients at the heart of all we do.

The role involves assessing patients in homes within the boroughs of Lambeth and Southwark, requiring the use of public transport; or via holding a current and valid UK driver's licence and /or vehicle that can be used for work purposes.

The prospective post holder must have a passion for rapid and holistic patient-centred community-based care, and demonstrated knowledge and experience and/or eagerness to play a pivotal role in admission avoidance. Confidence and higher-level capabilities in complex patient assessment and management; and being dynamic with your clinical reasoning to provide timely provision of interventions to support independence as much as possible is key. Someone with flexibility and enthusiasm for active participation in safe, effective and efficient patient care, service delivery, and staff development is highly desirable.

Main duties of the job

Provide expert Occupational Therapy and extended scope assessment, intervention, care planning and review of individual patients.

Demonstrated ability in working cross-professionally and in an extended scope capacity to support the @home services' commitment to meeting the patient's needs and delivering excellence in care.

Demonstrated strength in triaging referrals and prioritising timely allocation of urgent visits; and in supporting the therapy teams' daily operations within the duty therapist role.

Work frequently as a lone practitioner and at times in conjunction with other teams and colleagues, across a variety of locations within the community.

Facilitate and encourage multidisciplinary practice across traditional professional boundaries; liaising with health and social care professionals and voluntary agencies to deliver individualised and expert patient-centred care.

Supervise and line manage Band 6 Occupational Therapist and/or physiotherapists and Band 3 nursing rehabilitation support workers.

Lead on and support learning and development for the @home team, with refence to highlighting your Occupational therapy practice and knowledge to support complex patient cases and intervention planning.

Supervise, initiate and participate in evidence-based projects, departmental research and audit including recommendations for change in practice.

Opportunities for clinical and leadership skill development is supported through regular supervision with your line manager.

About us

GSTT @home is a hospital at home service covering both Lambeth and Southwark boroughs, and is a joint venture between health and social care. @home provides high quality, safe, intensive multidisciplinary acute treatment and monitoring in the patient's usual place of residence that would otherwise be carried out in hospital.

The service operates 7 days per week, with therapists working to as the standard shift pattern. With this post being part-time it is essential the post holder be able to work their hours flexibly and to contribute fully to both existing and new rota's.

Patients conditions range from those requiring a single episode of care postoperative to IV or antibiotic therapy; through to patients living with complex and long-term conditions, including the frail elderly, who may require periodic short-term intervention to maintain the best possible quality of life at home.

The service accepts referrals from GP's, LAS, Guys and St Thomas Hospitals, Kings College Hospital, St George's hospitals as well as other community services.

@home encompasses two main streams of work:

2-hour, same day or next day response to support early dischargefrom hospitals from Guy's & St Thomas, King's College and is integrated with secondary care.
