Job summary

Central London Community Healthcare Trust have an exciting opportunity for a part time Senior Community Nurse for CYP within the Brent Health Matters clinical service.

The Brent Health Matters (BHM) programme reduces health inequalities in the community by targeting hard to serve and less engaged members of our communities in Brent. The programme has been successfully supporting adults in the community and funding has now been made available to develop a new Children's health inequalities team within the programme to work alongside the very successful service for adults.

The successful candidates will demonstrate flexibility, excellent communication skills and the ability to work as team members as well as using your own initiative. This is an excellent opportunity to further develop your clinical and leadership skills and knowledge of reducing health inequalities for children.

The post holder should be able to work as a complex case manager, without direct supervision. You will be involved in delivering integration, working with GPs, social services and the third sector organisations.

You will be providing case management, clinical staff support, mentoring and clinical assessments for CYP who have difficulty accessing healthcare services and with some of the social determinants of health inequalities for the most vulnerable communities. You will be supporting the service at GP practices, community outreach events and where appropriate, supporting healthcare closer to home.

Main duties of the job

The service currently operates from - , Monday to Friday however the post holder will be required to work some weekends or evenings as the service will align with the wider BHM programme to deliver community engagement events.

The post holder will be a Registered Nurse, with community nursing/specialist practitioner qualification with relevant community experience of managing a complex caseload of patients. You will be responsible for a caseload of patients with complex health and social care needs. You will work autonomously to case manage these clients and use advanced nursing assessment skills, to manage their conditions in the will work as part of the health inequalities clinical team, working within and across professional and organisational boundaries.
