Therefore regular physical health support and monitoring is essential to level up such inequalities and promote longevity and quality of life for those we serve.

Main duties of the job

Be a visible, accessible and professional authoritative presence for patients and staff, working alongside staff in teams in direct clinical contact with patients and their families within the community to ensure high standards of physical health care are being carried out. To ensure provision of holistic nursing assessment and high quality, safe, evidence based care to patients. Assessing, diagnosing, care planning, reviewing all aspects of physical care and supporting mental health nurses and support workers in the community to also do this, upskilling staff when needed. To support the team manager in making strategic improvements to the health and safety of patients through CQUINs and institute of health improvement projects. The post holders will also be encouraged and supported to make improvements to care that they identify on a day to day basis. Help carry out audits to ensure that practise is safe and analyse data and formulate strategies to improve practice within the community teams with the staff.

Posts will be predominately based at the Kerrier CMHT base, however some cross cover will be required to help cover leave and to support workload commitments at each site, it would be expected that home visits would be made to some patients and therefore a car driver is essential for this post (mileage will be paid or use of base cars).
