To implement the Merton Short Breaks services for disabled children and their parents and carers.

Outcomes from this job will be:
Ensuring that the short breaks needs of severely disabled children and children with complex needs are met by local services

Support the Short Breaks Team Leader to ensure the involvement of disabled children and parents in service planning, commissioning and delivery is embedded in local practice

This post will contribute to the delivery of the Short Breaks programme in Merton.

The post holder will ensure that all referrals for short breaks are processed through the Allocations Meeting and support the referral, introduction and review of children into relevant short breaks.

The post-holder will hold a caseload of children and act as Lead Professional where relevant.


To work in partnership with children, young people and their families, team members and partner agencies to ensure positive outcomes are afforded to all young people.


Main Duties and Responsibilities


To ensure that the Short Breaks Programme is implemented in response to the identified needs of children with disability in Merton.


To identify the support needs of individual disabled children and young people to enable them to access short breaks across the programme.

To present findings to the allocations meetings via the Short Breaks Team Leader for budget approval.


To support parents and carers of disabled children to identify appropriate schemes and activities to meet the identified short break need for the children.

To support the introduction of the children to their allocated schemes.


To hold a caseload of children who are in receipt of short breaks and to act a lead professional as appropriate.

Being the first point of contact in order to co-ordinate the short breaks service provided for clients with a range of diverse needs.


To develop and maintain relationships with all professionals working with young people and families, for example: Other operational Social Work Teams (Children's Social Care and Youth Inclusion Division), Special Education Department staff, and other voluntary sector agencies with a family support role, Community Health Service professionals, Primary, Secondary and specialist Schools, Colleges.


To employ a variety of evidence-based skills and methods in order to achieve specific objectives as agreed with children, young people and their families and the short breaks Team Manager.


To undertake direct work with children, young people and their families, taking into account the wishes and feelings of children and young people.


The completion of high-quality assessments of need, using the CASA Assessment Framework, to include the gathering and analysis of information, to support the implementation of short breaks and to reduce the risk of family breakdown.


To work with children and their families in accordance with the aims and objectives of Merton's Childcare Strategy and the principles of the public law legal framework, for example the Children and Families Act 2014, Children Acts 1989 and 2004, SEN code of practice, short break regulations, Equalities Act, the Human Rights Act, Data Protection Act, etc.


To take account of a child's and their family's cultural, racial, linguistic background and religious affinity in providing services, within the overall framework of the Council's Equal Opportunities and Diversity Policy.

Working in a way that ensures user views and values are reflected in case planning and case work

To accept responsibility for efficiently managing an agreed workload and to ensure prioritisation and time management.

To support the Short Breaks Team Leader in the contract monitoring of Short Breaks providers.


To undertake direct work activities with children and young people to ensure that good working relationships are afforded, and that assessments and care plans are followed through.

This will necessitate flexibility in working hours e.g., occasional weekend working or at other times outside of office hours and include home visits.


To input data on CareFirst and other systems on an accurate and timely basis, and as such maintain accurate and up to date records including case chronologies, as required by the department within its recording systems, under the framework of the department's recording policy standards, using computers and other items of technology provided by the department.

To prepare high quality reports for Education, Health and Care plans (EHCPs), reviews and other purposes as required.

To attend and actively participate in team meetings and other forums and positively engage in training and team development.


To prepare for and attend supervision and appraisal and carry out actions as agreed in each session, under the overall framework of the Department's Supervision Policy.
