Primary Job Purpose
We are an independent private service with a team of eight, working alongside third

sector organisations, to promote and develop Person-Centred Services that wrap

around the needs of the Service User***You will be leading the team that supports a friendly, sociable, and talkative lady with

Learning Disabilities and Autism living in her own tenancy. Your role will be to ensure

that a team of professional carers, working on a one-to-one basis, provide high
- quality person-centred support which includes personal care, managing challenging

behaviours and administering medications. As a Team Leader your aim is to support

the individual to live her life in a safe, independent, and dignified way, while

managing the day to day running of the service. We are looking for a professional

who is committed to a person-centred approach and appreciates the importance of

facilitating choice and building skills and confidence. The position requires someone

reliable and trustworthy, wo can work from their own initiative and resolve conflict,

however it is essential that they are also friendly, kind and patient, conscious of

joining a professional service with structures in place to support, manage and

facilitate positive change, enabling team members to contribute to its development.

You will be the main contact on the frontline reporting directly to the SDO, while

regular team meetings, supervision and training contribute towards establishing a

successful working environment.

Job Description for Support Worker & Team Leader

Main Duties and Accountabilities:

Commitment to the person you support.
1) You will work with a level of autonomy and be skilled in enabling and sustaining

the rights of the individual to lead in the choices about how they live their life

2) You will support the individual to develop a lifestyle that is dignified, rewarding,

and fulfilling

3) You will understand and demonstrate the importance of being a strong community

connector, supporting the maintenance of existing relationships and development of

new relationships

4) You will work directly with the individual to provide a wide range of creative, open

minded and varied options to encourage inclusion, based around individual needs

and dreams

5) You will demonstrate ability and commitment to work with the individual through

their personal support plan to ensure they achieve their goals and outcomes and

celebrate their successes

6) You will recognise and acknowledge the importance of promoting the individual's

preferred communication style to develop successful and enjoyable relationships

7) You will recognise that all behaviour has a message value, working closely with

the person to understand what is being communicated and respond appropriately

8) You will recognise the value that the family has and the important role that they

play in the individual's life

9) You will help the individual to understand where there may be potential risks that

may affect them, at the same time helping the individual to understand how to be

safe

10) You will work alongside the person being supported to promote a safe and

healthy home environment in line with how they wish to live their life

11) You will be responsible and prudent in supporting the person to manage their

individual funds, for example by minimising your own expenses when supporting the

person to achieve outcomes

12) You will maximise opportunities to always promote and facilitate positive public

attitudes towards people with learning disabilities

13) You will work professionally and effectively within the team and with the

Guardians to ensure that the individual achieves their outcomes, and recognises the

value of outcome-based support

14) You will attend team meetings and make an active and positive contribution. This

will be done by providing sound, creative ideas and solutions to improve the

effectiveness of the team.

15) You will complete necessary paperwork, using all appropriate forms, tools and

resources and completing them carefully and correctly and at the right time. Ensure

that you read information written by others.

16) You will take on specific areas of responsibility for aspects of the work of the

team. The specific responsibilities for individual team members will be discussed and

agreed at team meetings.
