The Lead Thrombosis and Anticoagulation Pharmacist role will be essential for delivering the specialty pharmacy and clinical pharmacy team vision. This is an exciting replacement of an existing post, to provide leadership to the anticoagulation pharmacy team and clinical pharmacy provision to the cardiac pharmacy team.

The post-holder will manage and lead the adult anticoagulation pharmacy team at UHBW, basing themselves on the Bristol Royal Infirmary site.

The post-holder will expand on and lead all aspects of the thrombosis and anticoagulation pharmacy services, with a focus on new and important areas such as involvement in the inpatient anticoagulation dosing service, patient safety opportunities to deliver safe and effective practice relating to anticoagulation and thrombosis and work closely with the Trust venous thromboembolism (VTE) lead as part of the Trust-wide agenda.

Provision of expert pharmaceutical advice in thrombosis and anticoagulation to University Hospitals Bristol and Weston NHS Trust (UHBW) Ensure the delivery of anticoagulant postal dosing service to BNSSG STP catchment area patients. Lead and manage the pharmacy anticoagulation and thrombosis team Lead on the education and training agenda for the rotational pharmacists, technicians, foundation year pharmacists and the wider MDT Lead on Pharmacy elements of the Trust VTE agenda and steering group Deliver our late-duty (overtime) and weekend and Bank Holiday (off-duty) shifts on a rota basis, as outlined in the job description
