Overview
Aplos Health PCN was founded in 2019 and consists four practices in the London Borough of
Lewisham -Sydenham Green Group Practice, The Vale Medical Centre, Wells Park Practice and
Woolstone Medical Centre. We provide care to a patient population of 50,000.

In a little over a year and despite a global pandemic we have developed excellent working relationships between the practices.

We have run educational events for patients, started an adolescent and contraceptive clinic and are now looking forward to developing further innovative pathways and services for our patients.


Job Purpose


Health and wellbeing coaches support people to take proactive steps to improve the way they manage their physical and mental health conditions, based on what matters to them.

They support people to develop their knowledge, skills, and confidence in managing their health and care, to improve their health outcomes and quality of life and support them making changes in health related behaviours.

Health coaches do this by coaching and motivating people through multiple sessions to identify their needs and to set goals for themselves, and through providing access to interventions such as self-management education and peer support.


Health and wellbeing coaches manage and prioritise a caseload, according to the needs, priorities and support required by individuals in the caseload.

They identify when a person's needs are beyond the scope of the health and wellbeing coach role and refer them back to other health professionals or organisations.

Health and wellbeing coaches may work with people by phone, by video conference, or face-to-face - on a one to one basis and in groups

Health and wellbeing coaches are a key part of the PCN multidisciplinary team. They often work alongside social prescribing link workers and care co-ordinators to provide an all-encompassing approach to personalised care, and assist in the development and implementation of change to improve the service provision in the PCN

Responsibilities:

  • To coach and motivate patients through multiple sessions to identify their needs set goals and support them to implement their personalised health and care plan.
  • To provide personalised support to individuals, their families, and carers to ensure that they are active participants in their own healthcare and to empower them to take more control in manging their own health and wellbeing, to live independently and to improve their health outcomes through the following: a. Providing interventions such as selfmanagement, education, and peer support b. To actively signpost patients to the correct healthcare professional. c. Supporting people to establish and attain goals set by the person based on what is important to them, building on goals that are important to the individual d. Working with the social prescribing service to connect them to communitybased activities which support their health and wellbeing.
  • To refer patients who require the intervention of other healthcare professionals.
  • To provide support to local community groups and work with other health, social care and voluntary sector providers to support patients' health and wellbeing holistically.
  • To identify risk factors and categorise patients into appropriate risk banding.
  • To facilitate groups of patients with group consultations to assist patients to work with others to support their own goals.
  • To liaise with the practices and, where practicable, to standardise processes across the PCN.
  • To receive referrals and directed patients from triage services and other clinicians.
  • To maintain accurate clinical records of all patient consultations and related work.
  • To promote health coaching and social prescribing amongst the PCN and networked practices and to support colleagues to improve their skills and understanding of personalised care.
  • To support in the delivery of enhanced services and other service requirements on behalf of the PCN.
  • Manage and prioritise a caseload, in accordance with the needs, priorities and support required by individuals in the caseload
  • Strong awareness and understanding of when it is appropriate or necessary to refer people back to other health professionals/agencies, because the person's needs are beyond the scope of the health and wellbeing coach role e.g. when there is a change in the person's health condition or where unmet clinical support needs are identified.
