The post involves closely working with the Geriatrician in OPAU clinic.

The Older Person Assessment Unit is led by a part-time consultant Geriatrician in addition to an elderly Registrar who rotates in conducting Continence clinic in the acute.

The post also rotates in Continence Advisory Service across Lambeth & Southwark clinics, alongside contributing to the clinical governance framework of the service.


The role will play a key role in forging links with the Home delivery services to provide appropriate products for in-patients and out-patients and they will maintain a close working relationship with, and support colleagues in Primary Health Care Teams.

The post holder will conduct high-quality nurse-led continence advice clinics through either face to face, video, or telephone consultations providing expertise in a range of bladder and bowel problems.

The nurse specialist role will deliver specialist continence care within elderly care medicine and surgery in line with national recommendations across both hospital and community sites.

The post holder will act as an expert practitioner, resource and facilitator to educate and train other staff in the acute & community including students as well as hold the responsibility for managing a caseload of patients with complex bladder and/or bowel problems who require specialist assessment and treatment.


The post holder will be responsible for ensuring a rapid response service is provided to the wards in relation to bladder and bowel issues and delayed discharges.


The post holder will be responsible for contributing to the ongoing development of clinical practice, research and standards of care within the service, including the development of policies, protocols and guidelines and contributing to service developments in collaboration with multidisciplinary colleagues.


Also ensures the safety and well-being of service users, and always works in a manner that promotes dignity and human rights through the adoption of person-centred care principles.

Professional / Clinical responsibilities

To be professionally and legally responsible for all aspects of the post-holders work.


To carry out an assessment of patients as an autonomous practitioner, including complex presentations, and using clinical reasoning skills and knowledge of evidence-based practice, determines an appropriate care plan utilising a wide range of treatment skills and options.


To interpret and analyse clinical and non-clinical facts to form accurate diagnoses in a wide range of conditions including those with a complex presentation, with occasional guidance from specialist nurses.

To formulate accurate prognoses and recommend the best course of the intervention, developing comprehensive discharge plans.

To continually reassess patients in order to progress treatments effectively.

To coordinate intervention which may include other disciplines: advises and educates patient/carers/relatives/other health professionals.

To manage clinical risk within the caseload at all times.

To demonstrate physical ability to carry out specialist assessment and interventions including manual therapy techniques and therapeutic handling.

To demonstrate highly developed dexterity, coordination and palpatory sensory skills for assessment and manual treatment of patients.


To be able to manage potentially stressful, upsetting or emotional situations in an empathetic manner, including but not limited to those in Continence Advisory Service.

Management and Leadership

To be responsible for supervision and coordination of junior staff/students and assistants on a daily basis.


To be responsible for the operational management of the Band 6 nurse in the absence of the team leader or as required.

To flexibly plan and organise own time, patient caseload, meetings, training and adjusting plans as required according to staff.
