The Care Co-ordination Centre (CCC) is at the centre of the West Essex Health and Care Partnership transformation programme to support the delivery of the Out of Hospital Strategy.


  • Its ability to be effective and deliver the best outcomes for West Essex citizens relies on the CCC being informed and proactive in the following:
  • Holistic Overview of the adults previous care history to ensure the right care is facilitated
  • Understand the needs of the adult and the services in West Essex to support earliest possible discharge from hospital, avoid hospital admittance, reduce escalating need and support the rehabilitation of the adult
  • Multidisciplinary Team approach for complex adults led by the CCC
  • Integrated Social Care, Community Health and Mental Health within the CCC
  • Tracking of the adult through their transfer of care journey
  • Understanding of system capacity to make the best decision for the adult
  • Responsible for the sourcing of care
  • Incorporates the TOCH responsibilities
  • Manages system demand and informs future commissioning decisions
  • This is a 2 year journey but with a good start


Associate Practitioners are fundamental to the delivery of integrated care, working collaboratively with therapy, medical and nursing staff to support and facilitate the treatment of patients within their own homes.


The post holder will work as an active member of the Integrated Virtual Hospital and Urgent Community Response Team by providing high quality, efficient and effective care to patients with a focus on delivering care within individuals own homes supported with remote and assistive technology.


The post holder will assume responsibility for delegated aspects of healthcare delivery, working within a scope of practice under the direction/supervision of a registered practitioner.

They will work within relevant legal and ethical frameworks in accordance with organisational protocols and policies.

The post holder will be proficient, knowledgeable and competent. They will continue to develop in their role through the acquisition of further skills, knowledge and competencies, to be determined by the needs of the patients, the service and the team


The post holder will work as an autonomous practitioner within the Integrated Virtual Hospital/Urgent Community Response Team,supporting the delivery of care and working in alignment with the Care Coordination Centre (CCC).

The role will include collaboration with system partners and as part of the Primary Care Networks and the emerging Integrated Neighbour Teams (INT)as part of the out of hospital model of care

Here at EPUT we maintain our own bank of specialist staff to ensure that we are able offer safe and effective care at all times.

  • To support the work of the registered practitioners in the integrated virtual hospital/urgent community response team.
  • Maintain excellent communication with patients, relatives and members of the team demonstrating a variety in communication skills in accordance with patient needs.
  • Perform patient assessment (under supervision and after appropriate delegation from the registered practitioner). Assessments to include vital sign monitoring, history taking, mobility, tissue viability and nutrition.
  • Implement and evaluate individual care plans, utilising practical and theoretical knowledge under the supervision of a registered practitioner.
  • To carry out environmental assessments and consider need for equipment, including ordering and ensuring safe use.
  • Perform area specific clinical competencies such as phlebotomy, point of care testing and female catheterisation to enhance the delivery of patient care as delegated by the registered practitioner.
  • On the direction of a registered practitioner, refer patients to relevant professionals for specific treatment, advice or support.
