Stroke and Neurology Rehabilitation Coordinator

To triage incoming referrals for the Early Supported Discharge Service and the inpatient neuro-rehabilitation beds at St Margarets To develop links, and complete in-reach, with acute hospitals To communicate with the bed managers regarding outcome of inpatient referrals To contact all ESD patients within 24 hours of discharge To monitor the pathway of current patients To monitor and supervise Band 4 Associate Practitioner in their organisation and delivery of six-month review clinic. To maintain clinical records as per Trust and professional guidelines To continue to work clinically within professional standards and clinical guidelines, promote best practice, through active participation in continuing professional development (CPD) and to undertake all aspects of clinical duties as an autonomous practitioner To support service development and audit participation To update the team on incoming patients into ESD and Beech Ward to prepare them for assessments. To escalate appropriate concerns or queries To provide ESD patients with a comprehensive explanation of the ESD service and answer any queries that arise, being sensitive to their current presentation and concerns To support patients family or carers with concerns and address any queries they may have. To demonstrate good negotiation skills across a range of issues.
To deal with initial, verbal complaints sensitively, avoiding escalation where possible. To ensure that Trust and professional policies are understood and observed and that procedures are followed. To participate in, and contribute to, innovations in stroke and neurology pathways in West Essex. To represent the team To collect and provide data as required To plan and implement training To contribute to the development of team objectives or projects in liaison with peers.
To monitor stroke pathways and advise the service leads accordingly. To advise the service leads on issues of service delivery including shortfall, service pressures, etc. To be aware of other development of policies and standards in other areas of the team and contribute accordingly. To use specialist knowledge to guide triage decision making in line with inclusion and exclusion criteria A potential to transition to a 7-day service as per National Stroke guidelines and developments.
