An exciting opportunity has arisen within the Criminal Justice & Liaison & Diversion (CJLD) service, creating a position for a Band 5 RECONNECT Practitioner (Nurse, OT, Social Worker, Paramedic, Substance Misuse Worker) to join the team.

This role will be working across CJLD with a focus on the RECONNECT pathway within HMP Lincoln along with colleagues from Lincolnshire Action Trust (LAT).


RECONNECT in HMP Lincoln will support individuals with a range of health vulnerabilities to transition from prison into the community, removing health inequalities improving their engagement with health, social, third sector and mainstream services to improve their wellbeing.


This role requires an individual with relevant post registration experience working with people with complex vulnerabilities and reducing health inequalities.

Previous experience working in the Health & Justice arena is not essential but a keen interest in improving outcomes for inviduals in the criminal justice system is required.

This role would suit someone who is looking to start a career within this specialism or someone who is looking to continue building on existing skills and expanding knowledge already gained.


For this role enhanced prison vetting will be required as part of the recruitment pre-employment checks and prior to any contract being issued.


You will join a dedicated CJLD team who make a difference to patients, carers and the teams you work with.

You will have a keen interest in health and justice and a passion for supporting people as they are released from prison.


For RECONNECT this will mean you will work closely with key stakeholders within HMP Lincoln and the community to forge new care pathways, while being creative to meet individuals' needs.

Ability to work collaboratively is essential.


With the support of the Specialist Practitioner and working alongside colleagues in Lincolnshire Action Trust (LAT) you will work across HMP Lincoln responding to referrals to RECONNECT and building therapeutic relationships with individuals.

You will carry out assessments and identify multifactorial vulnerabilities necessitating access to community services on release from prison.

You will support the individual out into the community ensuring barriers to support are negated and accessed to meet the identified needs.


As RECONNECT is in development, there will be a great opportunity under the guidance and supervision of managers to be part of developing this pathway in directing and enabling new ways of working, which cut across traditional professional and cultural boundaries and developing relationships with key internal and external partners.

RECONNECT aims to support people leaving prison and will work in partnership with Lincolnshire Action Trust (LAT) and HMP Lincoln as well as colleagues within wider criminal justice system.

RECONNECT will accept referrals and undertake a screening /assessment to identify and agree the individual's particular health and social care needs and tailor a package of support to assist in accessing identified local services.

These services could include housing, General Practice, employment, or secondary mental health services and coordinating support in the community.


To work across the wider CJLD service, as required and in collaboration with the team manager, to include Custody Suites, Lincoln Crown and Magistrate Courts and Boston Magistrates Court in addition to core role within HMP Lincoln.
