Stroke Support Worker

The stroke support workers are an integral part of the stroke team helping to care for Stroke patients and their families.

As well as working along with the stroke team.


The support worker will support core stroke assessments including basic holistic information taking, initial risk assessments and baseline observations, including initial swallow screen*.

They will support basic clinical interventions such as taking samples (CSU, stool, MRSA swabs, blood), and can support referrals to specialist teams as required from initial assessment.

The support worker will support personal care delivery, facilitating an independent approach and supporting rehab plans where appropriate.

The support worker will support delivery of rehabilitation, as planned with the MDT and specific to the patient.

Where appropriate, they will facilitate self-directed rehabilitation activity and foster a solution focussed approach to promote future independence and self-management.

The support worker will follow care plans set out by Nursing/therapy teams.

This will be accomplished through attendance at MDMs and board rounds, and through supporting development of patient centred goals and interventions.

They will act as an advocate for the patient, and support the mental capacity process where appropriate, through co-ordination of best interest meeting
