Purpose of the role:

This role will provide an assertive outreach response to service users who are not currently engaged in treatment provision and recovery support and or require complex unmet need proactive engagement tools to facilitate access to services and reduce barriers to engaging in treatment. The role will work closely with housing support, homelessness services and Statutory providers of specialist clinical substance use support, and social care.

The focus of the role will be to provide structured psychosocial interventions via innovative and flexible engagement techniques to support harm reduction, case management and health promotion initiatives across Cardiff and Vale health and social care partners.

Engagement will be provided in a person centred and recovery-oriented delivery system. Working as part of an assertive street based outreach model, the role should aim to improve motivation to engage in services, and reduce dependence on crisis support and emergency services.

They will have excellent communication skills, be able to work as part of a multidisciplinary team, across agency cultures including those within statutory, private and voluntary sector partnerships. The post holder must be flexible and efficient, to ensure services are delivered equitably and with the service user at the heart of delivery.

They will have a non-judgmental approach to people who have recognised that their use has become problematic and be able to provide a welcoming and accessible service to existing and potential service users.

Individual supervision will be provided in line with Alliance partner organisational policy. Additional clinical support will be provided via peer supervision and review groups. Specific training will be available for the development of service provision in line with Continued Professional Development.

Key Duties and Responsibilities

To engage with a range of individuals with substance use issues who have adopted a ‘street lifestyle’ including people who are rough sleeping, begging, street sex working and street drinking with a view to engaging them in appropriate services and moving then into more settled lifestyles.

To participate in a pattern of shift work to enable maximum potential contact with individuals.

To participate in multi-agency initiatives including Case Review meetings and shared working to reduce the level of homelessness and its associated impact

To provide an assertive outreach service, supplying interventions and information regarding substance use and associated lifestyle behaviours that reduces the risk of overdose, reduces substance related deaths, reduces the spread of blood borne viruses, promotes healthy living, and minimises the potential risks and impact of substance related crime and disorder on the wider community.

Provide holistically support including support to access other non-substance use services, as well as direct assistance (e.g., with benefits/housing/primary care/dentists)

Work with and manage a case load of individuals who have been identified via the MDT.

Develop and maintain strong links with key stakeholders in local services. Liaise with individuals, statutory and voluntary organisations as appropriate and support service users to engage in additional health and social care needs.

To pro-actively work to increase motivation to access treatment and facilitate referral and entry into appropriate treatment.

To work closely with the CAVDAS RAPS, MDT counsellors and Harm Reduction leads to access support with Opiate Substitute Therapy (OST), alcohol clinical interventions, sexual health, contraception, vaccination, oral health, and dietary needs.

Work to reduce Anti-social behaviour and substance-related offending by liaising and working closely with the criminal justice partners to encourage service users to access treatment and build a positive recovery network

To work within agree policies, procedures, and protocols to safeguard children, vulnerable adults and those experiencing domestic abuse.

To attend and report to safeguarding reviews, meetings, and conferences such as Multi-Agency Risk Assessment Conferences (MARACs).

Undertake risk assessment and develop and review risk managements plans that seek to mitigate identified risk.

Deliver harm reduction initiatives including Naloxone training, needle exchange and Dried Blood Spot Testing

Enter data onto the CAVDAS and MDT case management systems and use the systems effectively to inform work with service users and concerned others.

Plan, facilitate and evaluate targeting outreach campaigns

To undertake any administrative function related to the role to a high standard, in a competent and timely fashion.

Contribute to the induction and orientation of new staff, volunteers, and students

Ensure professional development by seeking regular supervision and annual appraisals.

Effectively manage own time and prioritise workload, maintaining own administration and accurate personal records regarding travel, diary appointments, TOIL, and expenses incurred.
