The Borough Medical Director has an important role within the Integrated Care System, leading on the delivery at place level of complex changes and integrated working across health and social care.

This role requires strong, strategic leadership skills, critical reasoning, with the experience of stewardship of clinical colleagues, good governance and delivering impactful public outcomes.

NHS North West London Integrated Care Board (NHS NWL ICB) is a statutory body responsible for planning and allocating health and care resources to improve the lives of people in eight boroughs across North West London: Brent, Ealing, Hammersmith and Fulham, Harrow, Hillingdon, Hounslow, Royal Borough of Kensington and Chelsea and Westminster.

Job responsibilities

Working with the Place Based Director and Director for Primary Care to develop the place based partnerships and integrated neighbourhood teams whilst contributing to the delivery of the ICS primary care strategy.

Working to tackle and address inequalities and support population health management

Providing clinical leadership to ensure place works optimally, with a focus on reducing unwarranted variation, supporting the role of primary care to address urgent and emergency pressures and planned care, whilst developing primary care to deliver its core functions and enhanced services at scale through the implementation of the Fuller stocktake.

Working with the Medical and Nursing Directors for Primary Care and the Primary Care Quality and Improvement Leads to assure quality and drive improvement in primary care through shared learning and embedding the primary care quality agenda. (See supporting the primary care quality agenda section below.)

To be a primary care voice within the place based partnership and the NWL ICS to ensure that primary care contributes at all levels in realising the vision of the ICS

To act as a primary care connector, facilitating partnership working and managing the interface between NWL and the borough to delivery borough objectives

To work with the local Placed Based Director to set work plans, objectives and conduct regular reviews of outcomes for the local borough Primary Care Clinical Leads

To implement agreed ICS pathways of care; use guidance and feedback from NWL ICS Clinical Reference Groups (CRGs) and other clinical work-streams to support effective implementation of agreed clinical pathways in your borough

To support other local clinical leads (including PCN Clinical Directors) in their functions and leadership development within primary care, developing MDT neighbourhood teams and promoting innovation

To support and lead PCN CDs in the borough with quality improvement, delivery of agreed health outcomes and the adoption of population health management in practice

Coordinate with borough clinical partner to ensure support to borough and NWL medicines optimisation team.

To be a member of and contribute to the NW London Primary Care Executive Group, Local Clinical Interface Group/Senate and other groups/committees as the job requires

To attend the place based Primary Care Commissioning Committee as required

Represent NWL at Pan-London and Regional meetings where relevant

Support and/or provide clinical supervision and mentoring

Supporting the primary care quality agenda

Support the primary care quality agenda of the three elements of quality: patient safety, clinical effectiveness and patient experience within primary care.

Support the Primary Care Quality Improvement team to implement the primary care quality strategy by facilitating connections and forums to raise awareness and progress delivery.

Lead by example and share learning from managing serious incidents under the Patient Safety Incident Reporting Framework (PSIRF) and Learning from Patient Safety Events (LFPSE).

Work with the Primary Care Quality Improvement team to identify areas to incorporate quality improvement methodologies in primary care to ensure the provision of care to the right patients and service users at the right time by the right staff.

Support the broader social and economic development

Support the development of good clinical practice within primary care organisations across NWL

Provide relevant and timely specialist advice and guidance.

Work with key stakeholders to investigate the cause of any variance from plan/delivery targets and contribute to the implementation of solutions.

Communication and Engagement

Maintain clear and effective lines of communications at all levels with stakeholders

Be able to work effectively with services users, GP Practices, NHS and Non-NHS Providers

Support co-production as a way of working ensuring the key aspects of the work-streams with service users and experts by experience

Person Specification Knowledge
  • Awareness and understanding of the healthcare landscape in North West London and your borough.
  • Good working knowledge of educational support in primary care training, and health systems.
  • Understanding of national, regional and local primary care workforce transformation programmes.
  • Excellent verbal and written communication skills.
  • Excellent organisational skills with the ability to prioritise own workload and conflicting demands to meet tight deadlines.
  • Demonstrable leadership skills and an ability to influence and motivate others.
  • Ability to foster quality improvement within clinical practice.
  • Judgement: having the ability to understand and interpret clinical decisions for others and make informed judgements concerning clinical priorities.
  • Demonstrate an understanding of quality assurance activity.
  • Demonstrate the ability to hold to account clinical colleagues.
  • Ability to ensure robust governance processes are agreed and in place for projects.
  • Flexibility and willingness to travel within the borough and NWL area to provide widespread support to continuously develop the programmes of work.

Qualifications
  • Practicing GP in the borough with at least 4 sessions a month at a practice or PCN in the borough
  • Evidence of continuing professional development (CPD) and revalidation
  • Inclusion in the GMC Full and GP Register with a license to practice
  • Satisfactory Appraisal within the last 12 months or within the most recent 12 months of work, (if currently on a career break)
  • Clinical leadership qualification
Experience
  • Relevant clinical experience in primary care and leadership.
  • Experience of building productive and collaborative working relationships with internal and external stakeholders, working creatively toward shared goals.
  • Experience of reducing unwarranted clinical variation and the ability to foster quality improvement within clinical practice.
  • Evidence of planning and delivering of transformation change to deliver agreed objectives
  • Demonstrate an ability to lead and motivate individuals and teams.
  • Good working knowledge of professional requirements
  • Experience of project work focusing on benefits realisation and supporting transformational change.
  • Experience in liaising with NHS trusts, community organisations, medical schools and other partners and providers in leadership roles.
  • Experience of clinical and educational leadership and innovation, including managing a multi-professional team.
